Showing all 17EE was walking to tool-house, on recently cleared snow, slipped and fell on shoulder. EE went home that night with mild discomfort. Came in the next morning wanting to get checked out.
During loading of dollies onto the trailer, common practice to step over tongue of dolly to undo chains. While stepping over, EE tripped and landed on the trailer deck injuring a finger. Initial prognosis is broken finger.
Employee was exiting loader using proper 3-points of contact. Employee's foot slipped off last step of loader putting sudden stress on left arm and straining left shoulder.
Employee twisted knee while fueling truck due to uneven ground. Was seen by physician on 5/5/22. Diagnosed with sprained knee. Employee was released to work with no restrictions and no medical treatment was given. On 5/23/22 employee was seen again by physician. Was placed on restricted work duty due to continued issues with knee. Injury became reportable as on 5/23/22.
Employee was working on dredge. While walking on land near dredge, employee tripped over cable used to anchor dredge. Employee fell, landing on left knee causing injury to knee.
Employee was exiting Skid Steer Loader. Misstepped and came down hard on heel of right foot. Sprained ankle and deep tissue bruise to right foot.
Employee was working on sand plant screen box. While performing maintenance employee fell through maintenance door used to access lower portion of screen box. During fall employee struck elbow on edge of opening and jammed shoulder.
Employee was repairing engine on secondary crusher. Upon exiting the working platform employee tripped over extension cord, causing employee to lose balance and fall down steps, landing on the ground at bottom of steps. Fall resulted in bruising to employee's right side/back and two cracked ribs on right side.
Employee was working on a shot bench and twisted their knee.
Washing the plant shack with a power washer. Stepped backwards and tripped on the hose. Fell on right shoulder. Original injury report was 12/17/2015. The employee was under evaluation until it was determined on 02/01/2016 that the right rotator cup was torn and needed surgery. Thus needed to be reported as an accident.
Stepping into the manlift. He tried pulling himself in and stepped into basket when he slipped. His hard hat fell off as he continued up into the basket and hit his head on a piece of angle iron hanging under a plant conveyor.
Driver of stockpile truck was getting out of truck when his shirt sleeve caught on the door handle as he was getting out of the truck. Causing a muscle strain to right forearm.

TRYING TO BREAK LOOSE A DRAIN PLUG ON REAR AXLE UNDER THE LOADER WHEN THE DRAIN PLUG BROKE LOOSE. EE FELL BACK AND HIT HIS HEAD ON THE AXLE HOUSING.
SPLICING A BELT ON A CONVEYER WHEN HE SLIPPED AND FELL TO GROUND TWISTING HIS RIGHT ANKLE.
CLIMBING DOWN LADDER ON TRUCK, SLIPPED ON THE TOP STEP AND CAUGHT HIMSELF ON HANDLE AND HUNG THERE.
