Showing all 17Miner was moving a tool cart back to the shop from a work area when EE slipped on ice. EE was able to use the tool cart for support to avoid a fall, but EE's feet slipped out from under EE in opposite directions causing injury to the groin.

Employee was going up extension ladder leaned against a tank. The ladder slid when he reached about the 4th rung. The cleat of the ladder hit the employee in the right shin. He received 7 stitches and returned to work.
