Showing all 21Employee reported being at mud lake #4 at 8:30 am on 7/18 when he was attempting to stand an 8 inch PVC pipe (Pipe weight= 50 lbs) upright, when his left foot slipped backwards, resulting in a pull in the left groin area. Result after examination by Dr. was a left inguinal hernia.
Nurse called to lab and informed employee on bathroom floor for an undetermined amount of time, when she called supervisor for assistance, as she reported attempting to lift self off toilet, and resume standing position, when she felt bilateral legs tingling, she then stood up, twisted left ankle, resulting in fall to ground.
The employee states he was lifting a valve and tripped and fell on his knee. This was originally a minor injury but his knee has worsened and the employee is going to have surgery.
The employee was washing down his area and when he stepped back he tripped over the hose he was using. The employee fell onto a staircase located behind him hurting his shoulder.
The employee was stepping off of a stair case and sprained his ankle.
THE EMPLOYEE WAS WALKING THROUGH AREA 345 AND HE SLIPPED, FALLING INTO A PAN WITH CAUSTIC IN IT.

Ee was walking to the control room. He stepped off of the edge of the walkway.
Ee was hurrying and stumbled stepping on the edge of his foot. He fractured the bone on the outside edge of his left foot. Ee is off of work until injury heals
EE WAS WALKING WHILE LOOKING UP AND FELL OVER CENTRIFUGE HANDLE. HE FELL ON HIS RIGHT SIDE AND TRIED TO STOP HGIS FALL WITH HIS HAND. HE SPRAINED HIS RIGHT SHOULDER.
DRIVER WAS EXITING HIS 15C/Y DUMP TRUCK FOR INSPECTION. DRIVER DID NOT HAVE SECURE FOOTING. THE DRIVER'S FOOT SLIPPED FROM THE STEP & CAUSED HIMTO FALL TO GROUND. THE DRIVER RECEIVED MINOR BRUISES & SPRAINED BACK

CLOSING A REGRIND VLAVE, WHEN THE HANDLE SUDDENLY SPUN FREELY AROUND HARD HAT SLIPPED BACKWARDS AND HANDLE HIT EE ON TOP OF HEAD. SCALP LACERATION.
AFTER MAKING A SWITCH ON #27 PRECIPITATOR, THE EE WAS CLIMBING DOWN THE LADDER PLATFORM. WHEN HE STEPPED OFF THE LADDER TO THE GROUND HIS RIGHT ANKLE BUCKLED UNDER HIM. EE SUSTAINED SPRAIN TO HIS RIGHT ANKLE.
