Showing all 5Individual was fueling up D8 Dozer and finished fueling and was climbing down from dozer with fuel hose in hand and slipped and fell.
Conditions were clear & cold w/below freezing temps @27 degrees. Subject reached overhead to pull slack out of come-a-long and his footing slipped and fell backwards with hard hat falling off. As come-a-long came unattached hitting subject in head.
EE was installing new screens on screen #5 and reached out to grab a new screen and his feet slipped and his mid section landed against the spray bar.
Thw worker was washing the truck scale after starting the power washer he climbed over to the side of the scale and fell to the ground.
