Was parked in dump truck next to the shop ( not inside gated pit areas) He stepped out of his truck and walked along edge of shop where his foot got caught in a piece of belting laying on the ground. His left foot got caught and he fell down on both knees.

DURING WORK EMPLOYEE SLIPPED ON DAMP STEEL & FELL LANDING ON BACK & HIP. THOUGHT HE HAD KNOCKED THE WIND OUT OF HIMSELF. DIDN'T REPORT ACCIDENT UNTIL DAYS LATER. DIAGNOSED WITH CONTUSIONS TO BACK, HIP, RIGHT SHOULDER.
WHILE DESCENDING THE LADDER STEPS ON THE CAT 950 LOADER, EE SLIPPED, FALLING ABOUT 4' TO THE GROUND, AND LANDED ON HIS RIGHT SHOULDER. ALTHOUGH ACCIDENT OCCURRED IN MARCH 2001, IT WAS NOT REPO RTED BY EE TO MGMT. PERSONNEL UNTIL 5-31-01.
