While drilling a backstope with a long hole drill employee noticed a basket in the wire mesh. While re-positioning the drill the wire separated at adjoining seams allowing some small material (rock) in the basket to fall in front of the drill, startling the employee, causing the employee to retreat quickly and in doing so sprained left ankle in the process.

Showing 50 of 178After Jacking up a rail flat car to re-rail. 2 employees began to push car back on the track. 1 employee slipped causing both employees to fall & in the process the employee who slipped, boot contacted the other employee in the upper lip causing a laceration. Seen site medic & referred to ER, sutures were required, full duty release, no days lost, making it a medical reportable.
While preparing to off load a dumpster at trash yard employee slipped & fell on snow/ice in a non-designated travelway while attempting to chock vehicle. Employee was seen by site medic complaining of low back pain & was referred to the ER, X-rays were positive for a tail bone fracture & restrictions were issued.
Employee was operating a jumbo drill and when EE went to dismount the drill using the handrails and steps EE twisted left ankle as EE stepped to the ground. EE continued to work for about 2 hours but pain was persistent so EE contacted supervisor, seen by medic, referred to ER where restrictions were issued for swelling.
Employees low back/right leg had been sore for a period of time, and EE could not specify an event causing the pain. Exiting a mantrip at the start of shift EE re-aggravated the condition, and reported later in shift to the supervisor. EE was seen by the medic and referred to the ER and restricted until further care from a specialist.
A Non Employee (MSHA Inspector) was startled by an approved on-shift blast causing them to lose footing during their retreat and fell injuring right middle finger. They were seen by the site medic and referred to the ER where they were diagnosed with a dislocation to finger and required to wear a splint.
Employee was pulling a jack-leg machine & driver off a newly installed bolt when EE slipped causing EE to fall with the machine. EE's right ribs made contact with D-Handle. Employee reported to supervisor at the end of the shift and was seen by the medic who referred EE to the ER. X-Rays were positive for a non displaced fracture to a rib and restrictions were issued.
While stepping out of a 2.5 yard loader, employee lost balance causing EE to step backwards and the foot bent awkwardly. The following day, the employee was experiencing discomfort and EE was evaluated and diagnosed with a hairline fracture to the 5th metatarsal and issued restrictions.
Employee was walking around a parked loader and tripped and fell striking head on the ground. Employee was seen at the ER and received sutures for two lacerations on forehead. Full work release was issued, no days lost.
Employee states that they slipped in the parking lot while plugging in their personal vehicle block heater prior to their shift starting. EE stated that they caught themselves and did not fall, reported to the medics with lower back pain. Employee was sent to the E.R. for further evaluation and was given restrictions.
Employee was stepping back up on a Development Bolter to retrieve a needed wrench. In the process his left foot slipped off the bottom step of the bolter's access causing the employee to roll his left ankle when his foot contacted the ground. Employee was maintaining three points of contact. Employee was sent to the E.R. for an evaluation, x-rays were positive for a fracture.
Employee was moving/carrying a piece of cribbing & had both hands on it. While walking, he stumbled/tripped and started to fall; he put both hands out on the rib to brace his fall, which resulted in jarring to both wrists. X=Rays were negative. Restrictions were issued.
On 10/2/15, EE was carrying a valve & lost his balance. When he lost his balance, he contacted a handrail & valve he was carrying contacted his side/rib area (left side) resulting in a contusion. Took him to ER, x-rays were negative. Full Release issued. On 10/7/15, he said he "re-aggravated" it & he went to doctor. Restrictions were issued. (10/9/15-1st shift missed)
Employee was cleaning shotcrete off TBM-motor #1, when he stepped forward & twisted his right ankle. Restrictions were issued. 4 days missed. Full release issued on F/U appt on 4/1/2015. He RTW on 4/2/2015.
Employee was bolting with jackleg drill & lost his balance causing him to fall backwards. In the process, he grabbed the drill & got his L. ring finger pinched between the drill head & the leg on the drill resulting in a laceration & fracture to finger tip.
Employee tripped over his own feet while walking. He tried to catch himself on the canopy post and his right hand struck the post. Went to medic on site, then to ER. X-rays resulted in a metatarsal fracture. He was given a full release. On 1/27/2015 Orthopedic doctor issued restrictions~ then it became a Lost Time injury.
Employee was climbing a fixed ladder when he lost his balance, he grabbed the rung to regain his balance and experienced right shoulder pain. Doctor issued restrictions.
