EMPLOYEE WAS WO9KING ON LOADER MOTOR WHILE STANDING ON THE LOADER TIRE, HE THEN LOST HIS BALANCE AND GRABBED THE DOOR OF THE LOADER WHICH WAS NOT SHUT. THE DOOR CAME OPEN CAUSING THE FALL.
