The miner was working on installation of a new dust collector. He had to reposition his safety harness by standing on a "X" brace of the dust collector to adjust himself. He slipped on the angle iron of the "X" brace about 8 ft and his right foot hit the end of the brace. X ray revealed that he had fractured bone on his fifth (smallest) toe of his right foot.

A contractor employee was packaging material at his assigned bagging station when he slipped and fell on a wet spot on the floor where he was working. The treating physician diagnosed the injury as a right hand and right hip contusion. He was allowed to return to work on restrictive duty.

The employee was clearing out a product collector when he stood on a pipe then lost his balance. In an effort to regain his footing and catch himself on a forklift below, he aggravated his lower abdominal region. The doctor diagnosised the injury as a muscle strain in the lower abdominal area.

Due to a wet area on the floor because of rain, the employee's right foot slipped and twisted as he stepped out of the control room. He tried rest and applied ice on his foot right after the injury, but had to see a physician the next day due to increase pain. The attending physician diagnosed the injury as an ankle sprain and placed him on restricted work duty.

On Sunday, 12/18/05, the employee notified his supervisor he had had to go to the hospital emergency because his left knee was hurting due to an old injury. Four days later the employee told his supervisor he might have actually injured himself at the job site on 12/17/05 while stepping off a front end loader and will need an MRI to determine the severity of the injury.

EE WAS WORKING OFF AN EXTENSION LADDER WHICH WAS NOT TIED DOWN, LADDER SLIPPED OUT FROM UNDER EE CAUSING EE TO FALL AND CUT LEFT LEG IN THE CHIN AREA. THE WOUND, 5 STITCHES, WAS CAUSED WHEN EE SCRAPPED LEG ON THE LADDER RUNG AS EE WAS FALLING. EE ALSO RECIEVED MINOR SCRAPS AND BRUISES TO BOTH KNEES AS WELL AS RIGHT HAND.
