EE was helping to lift an empty water pipe, which was braced on a wooden block with a prybar between the pipe & block. As EE lifted the pipe, another employee kicked out the wooden block, causing the prybar to fall. EE stumbled and fell while trying to catch the falling prybar.

Showing all 14Employee was working inside the crusher underneath of the jaws. After exiting the crusher and shutting its door, he turned and slipped on the wet conveyor belt. He fell on the belt on his right side, injuring his right shoulder and arm.
Employee was standing on an I-beam of a drill rig. He was working on the rig when his slipped and got wedged between the radiator and intake tube of the compressor. His was holding onto to something above his head, and when he slipped it wrenched his right arm and shoulder which were in an awkward position.
