Subject stepped out of the mantrip and rolled his right ankle.

The subject was walking down the bath house steps. The second step broke and the subject twisted his foot.

EE was working on conveyor chain on miner. He was swinging a 4 lb hammer and slipped. When he fell he landed on his left side. He complained of hip pain.

Subject changing a belt roller. He stepped back and rolled his ankle.

EE WAS WALKING IN ENTRY, STEPPED IN MUD HOLE AND TWISTED LEFT KNEE.

EE CLEANING SURFACE BELT STRUCTURE, SLIPPED ON PIPE AND HIT LEFT KNEECAP ON STRUCTURE. RETURNED FOR NEXT SCHEDULED SHIFT. WORKED UNTIL 1/3/08. CONDITION HAD WORSENED OVER TIME REQUIRING SURGERY ON 1/3/08.

EE WAS WALKING THROUGH MUD AND HIS FOOT GOT STUCK TWISTING HIS KNEE. EE HEARD AND FELT A POP IN LEFT KNEE. EE TRIED TO WALK AND STARTED FEELING PAIN IN KNEE AND WAS TRANSPORTED OUTSIDE AT THAT TIME (8:30 PM)

EE STEPPED OFF A LEDGE NEXT TO THE RIBLINE ONTO THE SHUTTLE CAR ROADWAY INTO A MUDDY AREA, TWISTING HIS ANKLE.

EE WAS WALKING BELTLINE WHEN HE STEPPED ON UNEVEN GROUND, TWISTING RIGHT ANKLE/KNEE. AS THE DAY WENT ON THE PAIN INCREASED. ON 2-5-07 EE's KNEE WAS GIVING OUT AND HE WAS TRANSPORTED TO ER FOR EXAM.

Rock dusting the face area, walking backwards throwing rock dust and stepping in a hole, twisting his right ankle.

Employee was carrying a 4' x 8' x 1/8" sheet metal with 1 other employee. He was walking backwards when the other employee tripped, pushing the load onto him, causing him to fall backward dropping the sheet metal onto both legs.

Employee was shoveling on the end of the 3 north belt; his feet became stuck in the muck. When he tried to work them free he strained his knee.
