Showing all 3During morning hours with moderate lighting and overcast skies, employee was on the engine deck checking engine oil level. To better see the oil level, EE turned toward the front, missing step and fell approximately 7 feet to the ground, landing on back. The engine deck was clean and the ground was clear.
When pulling suction out of pond the right rear tire went up on the berm causing to much weight to front of the83 boom and the loose dirt landing sluffing causing the83 to tip resulting in a roll over EE then made the decision to unbuckle and jump causing him to land on his head resulting in a c2 fracture and a fracture to right wrist.
