EE was walking at the bottom floor of the plant, and slipped and fell. Discomfort in right knee. Medical released to full duty with self care. Several days later was referred to orthopedic due to discomfort, and MRI requested. Surgery not required. 3-31-2022 Physcial Therapy and restricted duty ordered by orthopedic.

Nov 2019, Ee was walking out of warehouse bay and stepped on uneven ground. Ee felt Left Knee pop, but did not fall. Feb 14, 2020...Ee had surgery on Left Knee as a result of injury.
Employee was descending stairs and foot slipped on stair tread. Employee caught weight on handrail and strained shoulder. Employee was checked by a physician and returned to full duty and has been working without restrictions until today. Today the employee went out for shoulder surgery.
On a wet/rainy day, EE slipped while dismounting a dump truck. EE felt a pull in shoulders as they attempted to catch themselves using the grab irons. EE had not had any missed days or restricted duty until now. EE required surgery on Rt Shoulder as a result of incident.
Employee was returning to lab after collecting a sample. As ee stepped off of concrete stair landing to ground, ee felt a pain in back of Right Knee. After several weeks of discomfort, employee had MRI that showed a need for surgery.
EE was walking from the change room to the electric shop before shift start and slipped, straining his L knee. The injury was treated conservatively but required out-patient surgery on 2/5/16.

EE was spotting as oper trammed bolter out of haul road. 4' steel stuck in chuck caught suspended feeder cable. Cable fell striking EE who fell to ground against bolter. Incident caused lacerations to scalp requiring sutures.

EE-1 was attempting to close a valve using a pipe wrench. While pulling with a EE-2, the wrench slipped & the two EE's fell backward to the floor. EE-1 landed on R. ankle & felt sharp pain. EE was originally diagnosed w/sprain & treated conservativley. on 8/12 MRI report indicates non-displaced fracture in heel.
EE was dismounting flatbed truck after releasing tie-down straps. Stepping onto flat unpaved surface- Twisted left ankle resulting in non-displaced fracture left foot.
EE was walking across a salt build up, stepped on a weak spot with his left foot. There was void underneath the weak spot, concealing a pool of hot brine. The weak spot collapsed and the employee's left leg entered the brine, burning the area between the knee and ankle.
Employee was walking along the north inside wall of the refinery. Employee slipped and fell. She turned to her knees to get up and hot brine spilled over the top and into the right boot. Employee sustained 1st. and 2nd. degree burnes to the right foot.

EE had just finished taking off covers of the miner to do p.m. work, as he finished up his rountine work he stepped backwards and tripped on the covers causing him to fall on his side.
EE was in process of lubricating the tail roller bearings, he walked behind the tail roller and slipped on spillage off the tail roller, landing on his upper body. EE treatment was non-medical until he submitted to surgery in October.

EE was climbing a ladder to tie the ladder off; the ladder shifted and started to fall. A spotter was holding the ladder but could not hold it when the ladder shifted. The ladder and employee fell approximately 12 feet to the ground, with the employee landing on top of the ladder.
EE was picking up old conduit on the ground floor of the screening plant. She was taking old pieces of conduit to a dumpster, her feet became entangled in wiring that was hanging out of the end of the conduit. She fell to the ground, injuring her back.
