EE was walking at the bottom floor of the plant, and slipped and fell. Discomfort in right knee. Medical released to full duty with self care. Several days later was referred to orthopedic due to discomfort, and MRI requested. Surgery not required. 3-31-2022 Physcial Therapy and restricted duty ordered by orthopedic.

Ee was walking down stairs and missed last 2 steps. Ee did not fall, caught self with Left leg. Ee did not immediately know that there was any injury.

Nov 2019, Ee was walking out of warehouse bay and stepped on uneven ground. Ee felt Left Knee pop, but did not fall. Feb 14, 2020...Ee had surgery on Left Knee as a result of injury.

Employee was descending stairs and foot slipped on stair tread. Employee caught weight on handrail and strained shoulder. Employee was checked by a physician and returned to full duty and has been working without restrictions until today. Today the employee went out for shoulder surgery.

On a wet/rainy day, EE slipped while dismounting a dump truck. EE felt a pull in shoulders as they attempted to catch themselves using the grab irons. EE had not had any missed days or restricted duty until now. EE required surgery on Rt Shoulder as a result of incident.

Employee was returning to lab after collecting a sample. As ee stepped off of concrete stair landing to ground, ee felt a pain in back of Right Knee. After several weeks of discomfort, employee had MRI that showed a need for surgery.

EE was walking from the change room to the electric shop before shift start and slipped, straining his L knee. The injury was treated conservatively but required out-patient surgery on 2/5/16.

EE stepped on a rock and fell catching fall with R arm. Shoulder pain treated conservatively and EE continued to perform job functions. Subsequent medical evaluation identified shoulder damage requiring surgery. First LWD 11/4/2015.

EE fell backward on a chair straining back. Conservative treatment applied. Medical follow-up resulted in medical deferment from work. First LWD 06/14

EE was barring down rib in his area, he stepped back and twisted his ankle.

Walking from power dist ctr to belt drive, EE felt "pop" in R knee. Pain increased after helping remove guard. Restrictions applied to next scheduled shift on 07/18/2014.

EE was crossing ground floor when he lost his balance and came down on his hands and knees causing small laceration to right middle finger.

Employee twisted his knee while walking into an excavation. He was released to full duty on 10/10/2013. MRI performed on 10/21/2013 showed a fracture. Employee was put on restrictions on 10/23/2013.

EE was spotting as oper trammed bolter out of haul road. 4' steel stuck in chuck caught suspended feeder cable. Cable fell striking EE who fell to ground against bolter. Incident caused lacerations to scalp requiring sutures.

EE lost balance stepping over 14" pipe and prevented fall with forward step of L foot. Placement of foot resulted in fracture of #5 metatarsal L foot. Temporary restricted mobility.

EE's left foot slipped through top mat causing foot to become caught between rebar and weight of plywoods. This caused a left foot fracture.

EE was walking toward transportation and twisted R ankle resulting in a sprain

Employee was stepping up onto form when he became dizzy, lost his balance and fell to the same level, due to high blood pressure. As a result of the fall the employee hit his head on a beam.

EE picked up repair parts to carry to vendor truck, stepped back against a supply cart and lost balance falling against a portable stairway structure. Pinched R little finger between part and stair frame--laceration required stitches.

EE was walking to transportation and fell face-first to the ground because of a non-work related medical condition. Impact caused lacerations to the face that required sutures. EE is being treated for non-work related illness

EE dismounting loader, twisted R knee and fell to ground. Treated as First Aid. On 3/2, Co notified MRI revealed torn meniscus & ruptured patellar tendon. EE issued work restrictions. Subsequently terminated employment on 3/23.

Received medical notes on 09/27/11. EE was working in a pipe rack connecting steel when he lost his balance and fell, caught by his harness and lanyard.

Employee fell and hurt L/shoulder on accident date. Continued to work regular job until 10/15/2011 Lost time started 10/17/12011

EE Stumbled on crack in concrete deck as he was retreating from spillage from a pump that was draining unexpectly - EE fell forward into hot brine burning both arms and (from splash) face and neck>

EE-1 was attempting to close a valve using a pipe wrench. While pulling with a EE-2, the wrench slipped & the two EE's fell backward to the floor. EE-1 landed on R. ankle & felt sharp pain. EE was originally diagnosed w/sprain & treated conservativley. on 8/12 MRI report indicates non-displaced fracture in heel.

EE was dismounting flatbed truck after releasing tie-down straps. Stepping onto flat unpaved surface- Twisted left ankle resulting in non-displaced fracture left foot.

EE was walking across a salt build up, stepped on a weak spot with his left foot. There was void underneath the weak spot, concealing a pool of hot brine. The weak spot collapsed and the employee's left leg entered the brine, burning the area between the knee and ankle.

Employee was walking along the north inside wall of the refinery. Employee slipped and fell. She turned to her knees to get up and hot brine spilled over the top and into the right boot. Employee sustained 1st. and 2nd. degree burnes to the right foot.

Grating was removed for clean up and as employee was pulling wire, he slipped and fell into a ditch.

Employee was in process of moving continuous miner cable loops with a rope from long room on the left to cut ribs for survey sights when he slipped and fell down to the floor fracturing his right lower tibia and ankle.

EE was walking to a safety meeting, walked into a drainage culvert and fell on her knee, causing the injury. The injury was minor, a contusion to the knee.

EE reports that he slipped on mud at base of stairs near old floatation control room and tweeked his back. On 10/30 EE casually relayed possible back injury to non-WC med provider. Upon learning this, investigation conducted and comp accepted 11/6.

EE was climbing up side on a dump truck. The area around truck was muddy. As Employee ascended the step, EE's feet slipped and fell backwards off the step. The employee grabbed an I Beam to prevent the fall and Temporarily dislocated his R shoulder.

EE was descending a stable 8' stepladder set on flat concrete surface. EE reports thinking that he was stepping to ground level, but was one rung above. Fell backward striking L shoulder on ground. EE returned to work under medical restrictions.

EE stepped back towards jeep from inside drive area, his foot slipped on a piece of belting on the roadway, causing pain to his right knee.

EE had just finished taking off covers of the miner to do p.m. work, as he finished up his rountine work he stepped backwards and tripped on the covers causing him to fall on his side.

EE was in process of lubricating the tail roller bearings, he walked behind the tail roller and slipped on spillage off the tail roller, landing on his upper body. EE treatment was non-medical until he submitted to surgery in October.

EE was removing old lighting conduit, using a 10ft ladder. He was wearing fall protection, tied off to a structure in front of him, he could not tie off overhead. The conduit came out of its brackets, striking him on the chest and knocking him off of the ladder. The length of his lanyard allowed him to fall to the floor. He landed on his feet, spraining his left ankle.

EE was climbing a ladder to tie the ladder off; the ladder shifted and started to fall. A spotter was holding the ladder but could not hold it when the ladder shifted. The ladder and employee fell approximately 12 feet to the ground, with the employee landing on top of the ladder.

EE was picking up old conduit on the ground floor of the screening plant. She was taking old pieces of conduit to a dumpster, her feet became entangled in wiring that was hanging out of the end of the conduit. She fell to the ground, injuring her back.

EE was walking to his jeep to get a tool that he needed and while doing so he stepped on an old track rail and twisted his right ankle. The jeep was located at the 6th level.

Employee dumped load onto feeder, EE preparing to leave the feeder,power cable hung up between guard that keeps the cable away from the car.guard was hanging away from the car. employee stepped off the cable, cable reel took up slack as he stepped over the it.employee caught his foot on the cable fell to the ground.

While Employee was pulling water hose out of ram car travel way the employee tripped on a pile of muck. In attempting to break the fall the employee stretched out her right arm landing on her right hand dislocating the little finger.

Employee came down stairs, when he reached ground level, he turned to go to maintenance truck and stepped into a crack in the floor pad with the outside of his foot and twisted his right ankle.

Employee reports that he slipped and fell on the loadout stairway and injured left knee possible connective tissue damage. Employee notified management on 8/27/2007.

The employee was in the process of loading a railcar, he had just finished loading on end of the car. He was in the process of changing loading ends,he went to retrieve the safety lanyard on the southeast end of the car. Lost his balance and fell to the ground.

EE was leaving control room at end of shift with large bag of trash and personal items in his hands. While descending steps, his foot rolled forward losing his balance and falling to the midway landing. Initial Dr. Visit on 1-31-07. No restrictions given. Restricted duty at follow-up visit on 2-16-07. Misc Mail 10/9/07 #15-9/11/07 with restrict days 87.

Employee sprained left ankle while exiting a front end loader. He stepped onto the ground with full weight on left leg, foot turned and sprained ankle. Restricted Duty.

Employee was descending a set of 3 steps. He tripped and fell into the handrails of another set of steps, breaking 3 ribs. Started losing time 4-14-06.
