Performing maintenance on the 1A conveyor. Propped the step ladder against the tower holding up the tail of the conveyor. Ladder was on uneven ground. Climbed to the 3rd step (36") placed left foot on the tower, step ladder kicked out and she fell to ground.

Climbing down ladder from loader. When he stepped to the ground he felt a pop in his knee. Went to the doctor on 6-26-13. X-ray negative. Went back to doctor on 7/1/13 - taken out of work for another week - Rtw on 7-8-13.

Cleaning up dirt around plant. There was a dirt ridge under his feet slipped and fell on knee and injured shoulder.

Putting the window down in the crane. Bees flew toward employee. He is allergic to bees so he jumped off the crane and hurt his left knee.
