Operators feet slipped/missed step while descending the steps of a articulated dump truck. Truck was parked on level ground in dry conditions on a sunny day. Factory hand railing and steps in place and in proper condition. Operator had "pump knot" on forehead.
Truck driver climbed up on the right front fender of EE's coal truck to add motor oil to the motor, slipped off of fender landing on EE's right side of EE's head and right shoulder. Had pain in EE's shoulder and back.
Said EE slipped on Rock and fell on EE's right side. When EE landed EE hit EE's ribs. Pain in EE's chest and right ribs. Was having trouble breathing.
Driver went to step out of the truck and slipped off the bottom step.
Employee had just arrived on west section and was down on knees waiting to begin work, when employee stood up employee's body twisted and employee fell to right side, which resulted in a broken fibula in right leg.
Driver stepped up onto tire to clean windshield and slipped & fell breaking wrist.
Employee was trying to repair the stab shaft on the South Mains Feeder. EE was attempting to pry the sleeve off the stab shaft. The slate bar slipped causing ee to slip and twist right leg.
Employee was sitting down with sledge hammer busting rocks out of the scalping screen, as employee went to stand up employee slipped and twisted left ankle. Employee was taken to Elkins hospital where they conducted an X-ray which showed that employee had a fractured bone in left ankle.
This injury occurred back on 7-23-18. The employee later had complications because of the incident and was scheduled for knee surgery on 12-21-2018. The initial incident occurred on #2 beltline, where the employee stated EE slipped on slick bottom and felt a pain on the inside of EE's right knee.
Employee was at 5 drive removing lights from the roof and tripped over cable lying on the floor. Employee twisted right knee.
While working on the feeder, ee stepped off of the feeder and heard a popping in knee. EE had immediate pain after ee heard the pop.
Employee had set the CM up in #6 entry. EE was turning EE's body around to get better positioning, as EE turned EE tripped over a lump of coal and fell to the ground. this resulted in a fracture to EE's right elbow. EE has since had surgery to correct the fracture in EE's elbow.
On June 29, 2016 at approximately 10:10 PM a foreman reported that they saw an ignition in the crusher area of the Cogar Feeder/Crusher Serial #10-035 located on 1 Section at the tailpiece of #16 belt. The material in the crusher at the time of the alleged ignition was sandstone.
At approx.10:00PM on Section 1 in the #5 entry a section foreman knelt down to nail a back check down. While knelt down EE notice a piece of equipment coming through the back check. The foreman yelled and lunged to get out of the way and fell down. The foreman complained of pain in lower left leg. EE was transported to St. Joseph where an x-ray showed a fractured Tibia.
At the end of his shift the employee was crossing over the bridge and lost his footing and fell. This caused his leg to be pinched between the bridge and Lo-Lo causing a contusion. The employee was treated and released and remains off at this time.
The employee was cleaning a rock box at 2A head drive when he fell onto the # 11 belt after it inadvertently started due to not being blocked from motion. The employee was diagnosed the hairline fracture to 3 vertebrae's in his neck.
Employee was duck walking in the #3 entry when he tripped on a rock, which has been scaled down from the top, and fell to the ground breaking his leg.
The employee was walking to the stacker belt when he stepped in a hole and hurt his left knee. X-rays taken at Davis Memorial Hospital were negative and the employee returned to work on his next scheduled shift. The employee continued having pain and was seen by an orthopedist, which recommended surgery on Monday July 7, 2014. *EE was operated on 7/7/14 and will be off work.
Was sitting on picnic table. When he got up he lost his balance and grabbed wall to keep from falling and hand landed on a screw in wall and cut his hand.
Pulled truck off scales went back to scan card. Right foot slipped on step while holding handrail. Fell and twisted left knee and pulled right shoulder.
EE was walking to mantrip on down hill grade and slipped on pavement and fell on his rear-end.
Was crossing bridges and fell striking knee on pavement. Injured on 6-7-10 and returned to work 6-8-10 and worked until 7-28-10. Now he will be off until after surgery.
Was running to answer mine phone and slipped on wet pavement and broke his right leg, requiring surgery.
EE was pulling belt with channel locks and changed locks, slipped and EE fell into rib and hurt right shoulder. EE had to have surgery on 12/8/08. Returned to work on light duty 12/9/08.
Lost footing in mud and fell. One foot was too deep and didn't give when fell.
The injured was walking around the bucket of the parked endloader in order to get in the machine when his right foot slipped on a patch of ice in the roadway and his left foot remained stationary. This caused his left foot/leg to twist and resulted in his full body weight to shift to his left foot/leg causing a fracture just above the ankle.
While walking in the haul road entry of the 3 Section construction area the injured stepped into a hole and twisted his right ankle causing him to fall with his full weight resting on his ankle. The area was wet and muddy.
Employee was installing a belt wiper. He slipped catching himself, but striking his right shoulder - breaking his right clavical.
The employee was walking across a floor that was being prepared for concrete, he tripped on the reinforcing wire mesh. The trip caused him to fall striking his knee on a metal curb - requiring 6 sutures.
