IE was fueling up haul truck with DEF fluid, as IE went to place the nozzle back IE's foot got wrapped with the hose. Causing IE to stumble when IE stepped forward causing pain in right knee. Took IE to clinic, was fully released. IE was still in pain after a week of first visit so we took IE back to the clinic on 1/18/2021, that's when IE received restrictions.

Employee was operating a skid loader and parked it. As he exited the skid loader he stepped onto the top of the grounded bucket. His foot slipped and he twisted his ankle.
