Employee was exiting cab of locomotive and while stepping out on the two steps that stepped down to the cat walk he did not get solid contact with the 2nd step and twisted his leg. He grabbed the rail and stopped his fall but his leg hit the catwalk with alot of force...breaking the bone below the knee in 2 places.

UPON LEAVING, THE DRIVER TOOK A SHORT CUT DOWN THE SIDE OF THE RAMP INSTEAD OF WALKING TO THE END OF THE RAMP AND AROUND TO THE CAB, AND FELL.
LOADER OPERATOR WAS PARKING FOR THE DAY AND NOTICED THE WINDSHIELD WIPER WAS NOT ATTACHED AS IT SHOULD BE. EE STEPPED FROM THE DECK OF MACHINE TO THE LEFT FRONT FENDER TO MAKE CORRECTIONS. UPO N ATTEMPTING TO STEP BACK TO DECK SLIPPED AND FELL TO THE GROUND.
