EE was in the process of fueling up an excavator at the North Fuel tank area on the refuse disposal area. When the EE stepped down off the track to the ground, their left knee gave out. The EE continued to work until 2/13/2026 when they went off for surgery on their knee.

IE was washing underneath the belt line at the dewatering system. During this time IE tripped on the water line and lost balance. IE fell and cut right forearm and received 4 sutures.
IE was pulling 2x12's out from underneath the cribs and stepped backwards. IE's heal hit a crib and EE fell back and landed on EE's scsr. IE was diagnosed with two broken ribs on EE's right side.
Employee was setting up to do some maintenance on the feeder during the belt move. The belt crew shut off the 6" waterline 10 x-cuts outby the feeder. When they took apart the Victaulic fitting on the 2" water line. It recoiled backwards striking the IE in the back and IE fell to the ground with IE's leg folding behind IE. IE was taken off work pending MRI to the knee.
IE was watering the roads on the NE-mains travel road. IE got out of the tractor to hang up 1.5" hdpe pipe that IE knocked down. When IE went to hang the lines up IE slipped and hit head against the fifth wheel. IE received 5 sutures and an orbital fracture.
IE was carrying trash to the truck. IE tripped over a cable bolt laying on the ground. IE fell and tried catching self with arm extended. IE has a rotator tear in the left shoulder.
IE was rebuilding a stopping at x-cut 45. Area was wet and muddy. The wall was approximately 10' when the wall started leaning towards towards IE. IE felt the wall falling and jumped off the 4' ring of a ladder. IE had block land on EE's right leg. IE was diagnosed with a sprained wrist and a contusion to EE's right lower leg.
IE was standing on shield 5 pulling in shields. After the shields had been pulled and pan pushed forward. IE stepped backwards off shield 5. IE stepped into a hole that was 4-6 inches deep. IE rolled an ankle. IE was diagnosed with a sprained ankle.
IE was washing the slope at the 2200'. EE had the fire hose on top of shoulder and was transitioning it to under EE's arm. The fire nozzle caught EE's bibs or hit EE's hip causing it to turn on. The IE lost balance and fell to the ground striking EE's head on the floor. Witness thought EE was knocked unconscious but IE didn't believe EE was. IE was diagnosed with a concussion.
IE was sealing a stopping on HG-16 x-cut 68 between entry 2 and 3. IE fell from a 6' ladder two different times. The second time EE hit EE's head. IE was diagnosed with a concussion.
IE was dropping ventilation tubing in the second open next to a sump. When IE dropped the tube IE had their body between the sump and the ventilation tube. One of the tubes was about to come down on IE so IE reacted and stepped back. IE fell in the sump. Fell approximately 5 feet on to back. IE was wrote off work with contusion/soreness.
IE was standing on a ladder on the opposite side of the stopping being built. The wall was almost to the point it could be capped off. The wall fell over and knocked IE off the ladder. IE landed on top of the block. IE was diagnosed with a lower right leg (calf area) bruise/contusion.

IE was carrying a monster shaft down the longwall face. IE was stepping over a shield foot when IE tripped and fell to the ground. The monster shaft landed on IE's finger. IE received 5 sutures to the ring finger on left hand.
IE was carrying buckets of broken concrete down the stairs and slipped/twisted EE's knee. The stair case the IE was carrying the broken up concrete was near the top of the plant. IE was diagnosed with a torn meniscus.
IE was using a water hose to fill the water trailer. IE lost control of the water hose when opening the valve. IE lost balance stepping off the water trailer and twisted EE's ankle. This resulted in fracture to EE's right ankle.
IE was walking from the #3 Entry face and tripped over some gob. IE twisted left knee. IE continued to work until 10/4/2021. Removed from work by medical on 10/5/2021 pending MRI. Safety dept notified on 10/11/2021 of this action.
IE was going to the section Power Center to unlatch a power receptacle. IE tripped over power cables entering the area of the PC. IE fell causing a contusion to left shoulder. A follow visit to a medical professional revealed IE has partially torn the bicep tendon.
IE was injured unclogging the rock dust hose. While holding the dust hose, the hose dislodged the dust causing EE to fall into the stage loader panic bar. This struck EE across the back causing a fracture to EE's lower lumbar.

Injured employee was walking by refuse vessel screen and stepped off a 3-4 inch drop off and "tweaked" middle back. Reported as a file only initially and modified to an LTA on 08/02/2018.
IE was preparing to do maintenance on the A-side scalper screen. EE was entering the screen from the high side. EE foot slipped out causing a hyperextension of left knee. Initially a file only. IE received medical treatment on 05/02/2018.
Contractor employee was shoveling the belt at TG4 xc 7 when the employee lost their balance. The employee attempted to catch self and inadvertently contacted a top conveyor idler. This contact resulted in a laceration to left index finger. This laceration required stitches.
IE was getting into the A-Side refuse vessel screen when EE felt a pop in right knee. IE received medical treatment on 6.21.18.
Injured employee was attempting to adjust sight lasers. IE knocked the breaker to the continuous miner and used a drum bit lug as a step. As IE stretched up, slipped and left leg was lacerated by the miner bit. This laceration required stitches.
While working with the underground pumpable crib crew, the employee fell and cut their hand on a rock. They received 5 stitches and returned to work for their next regularly scheduled shift.
Employee was packing 2-crib block when employee tripped. Employee fell striking employee's mouth and right front tooth against a crib block resulting in a chipped/broken tooth.
Employee's right foot became stuck between the relay bar and the toe of shield 140. EE reached over their back towards the next shield and lost balance. Employee fell over fracturing EE's right leg.
An employee was attempting to crossover a pressurized water hose, when the hose burst. The employee then slipped/tripped over a 2" waterline on the ground. This resulted in a sprained/twisted ankle.
Employee arrived to work and entered the bathhouse through the boot wash area. He was ascending the steps. As he stepped onto the concrete floor his foot slipped and right knee buckled. As he fell to the ground he stated he felt his knee cap "pop". He was assisted into the office area for treatment.
EE right foot became stuck between the longwall shield foot and the pan line relay bar. EE lost his balance and fell over, fracturing his lower right leg and dislocating his right ankle.
Plant employee got flock on his boots then proceeded up the stairs. As he approached the top of the stairs he slipped and fell onto the top landing. Causing a fracture to his left ribs.
Employee was loading belt structure onto a trailer, when he slipped and hit his ribs on the skid steer.
