Showing all 7Due to cooler weather, plant having problems with belts, pulleys and material freezing up. Conveyor belt started tracking hard to south side. EE was looking at tail pulley for material build up and slipped, twisting EE's ankle.
EE was checking the sight pole for an excavator operator. Some of the stone below his feet gave way and he fell into the ditch. He continued to work the rest of the day saying he was fine. He decided to go to the doctor on 4/22/2011. EE is part-time and any time off was not related to this injury.
Two workers were putting together 6" diameter yelomine pip. EE got his foot stuck in a strap, which caused him to trip, twist and fall on the ground. The resulting injury was a broken fibula in ee right leg.
Employee was attempting to wash out the bucket on the 980 Wheel Loader he was operating and while pulling on the 1 1/2" wash out hose, lost his footing and twisted his ankle. He was taken to the hospital for treatment. Upon inspection of the area, the ground apppears relatively flat. The weather was sunny and dry. The ground may have been wet from spraying the bucket out.
Stepped on loose ladder on left rear of the machine and fell to the ground.
Lifting the ROPS from the piece of equipment. It sprang free, striking the mechanic and causing him to fall (93") to the ground.
