Isolated case of Employee misconduct where Employee failed to follow procedures and climbed onto the sand screw containment at the Wash Plant without the use of a ladder or a fall protection system and fell approximately 8.5 feet to the ground landing on his left shoulder.

Employee was climbing a dirt embankment instead of using the ramp. He twisted his left foot and complained of a sharp pain in his heel. Employee asked to see a doctor 8/22/07. Note: due to conflicting stories of what body part is injured, this injury is being questioned. This injury occurred one week after a previous injury and one week after his hire date.
