Dismounting grader missed first step lost their balance and fell to ground on right shoulder. No restrictions and able to continue job full time. On 11/19/2021 notified that surgery was to be performed.

Showing all 7Employee was tying in two headings for blasting and his right knee 'let go' and as he fell to the mine floor he tried to catch himself. Employee injured left wrist and right knee. Employee has had previous incidents with his right knee.
Local customer slip and fell on an ice patch near the load out yard, which resulted in a break of left leg above ankle. The customer (owner/operator) stopped at a section of the yard with an ice puddle even though most of the yard was clear and dry. The driver was observed failing to recognize the ice patch. He reported knowing that he needed to use caution.
A loader operator twisted his left ankle when dismounting the left rear steps of a Cat 988H wheel loader. There was loose rocks near the bottom area of the ladder that he potentially stepped on. The loader operator had gone to the cab for his lunch box for an early lunch break.
Employee was loading shot when stood up right knee popped out of place. Employee reported this is not the first time his knee went out, happened in previous employment. MRI done on 7/10/12 showed weak Patella, employee was given a brace and set up for physical therapy. Since incident employee has been able to work within his underground miner 1 classification.
The plant operator was carrying two 5-gal buckets of lube oil to a cone crusher lube system. The operator shared that he was not paying full attention to his travelway when he stepped in a vehicle rut and twisted his right knee. The operator completed his shift, requested medical treatment the following day. An MRI and follow up reviled a sprained ACL.
HVAC contractor was applying caulk to the outer exhaust fan shield from a ladder. The ladder went out from under EE and his right index finger caught the sheet metal flashing around the fan shield. First aid was applied at the sign in room and his fellow employee drove EE to the Hospital where he received ten stitches and a referral to a hand physician.
