Employee was getting off the bus at the Underground to report for pre-shift lineout. Upon exiting the bus, the employee twisted their right ankle.

Warehouse leadman was assisting move ground engagement tools onto a pallet using a lifting magnet. When EE released the magnet and stood up, EE lost balance/slipped and fell, resulting in an injury to shoulder and knee.
Employee was walking a semi-cleared footprint for a future embankment and tripped in a small hole. They twisted an ankle and was taken to the site EMT and received first aid for a sprain. After traveling home, employee visited a clinic and a break in their fibula was identified.
Equipment operator felt a "pop" in knee when exiting the operator's haul truck. The operator has been placed on restrictions.

A janitorial employee was exiting the administration building when EE lost balance and fell because of a small rock that was underneath the rug. This resulted in a minor knee injury.
The Employee was walking into work (around 6 am). EE was carrying lunch box. EE stepped off the edge of the sidewalk. This caused EE to roll an ankle (no injury) and fall onto left side (shoulder was dislocated). EE fell just past the entrance to Haile's admin building (near the turnstiles).
Process Maintenance Tech prepping to start a repair on CIL tank 4 launder, prior to repair work starting, the inflatable plug which was just installed in the upcomer ruptured, causing one maintenance tech to have slurry enter their eye and received a fracture to the ankle.
Process Technician was walking up a flight of stairs when their foot got caught on a step. Employee tripped, fell forward and tried to slow their fall by grabbing onto the handrail. Their left hand slipped off of the handrail as they fell forward, hitting their knee and shin. The employee felt pain in the right shoulder area after recovering from the fall.
Maintenance technician dismounting DT4 dozer with hose in hand, technician's hand then foot slipped causing the technician to fall approximately 5ft from the track to the ground, landing on the back.

A shoulder injury occurred causing restricted duty on a mobile maintenance employee. The technician was pressure washing a blast drill when EE stepped into a sampling port on the drill deck. The technician tried to catch/brace self thus causing injury.
Employee was walking towards a water pump to check fuel level when employee lost footing and tried to catch self with hand on the pump. The employee cut finger on the pump when employee tried to catch self. As a result, the employee received three sutures in the right index finger.
IE was standing on a wooden block while fueling a generator IE slipped off block and felt a pain in IE's back.
While on break in the designated smoking area, employee was standing, took a step backward, and felt right knee pop, causing employee to fall to the ground. Employee was later diagnosed with a knee sprain.
While stepping across a pipeline 42 inches high, employee slipped, striking head upon the pipe flange, requiring sutures. Employee was not scheduled to work after this day because it was the last day of a two day shutdown job.

The normal walking path was under construction. The employee was not yet clocked in and was not performing work at the time of injury.
While unloading delivery truck, employee pulled on a pallet strap to reposition it. The strap broke, causing the employee to stumble backwards, falling off the rear of the van to the ground below. Employee returned to work but later complained of back pain. EE was examined by a physician on 11/3/16 and placed on restricted duty due to back pain and sprained ligament in back.
Employee stepped into a hole adjacent to a recently poured concrete pad. This action resulted in employee breaking left ankle.
EE stepped in a divot in the ground and knee gave out. Physician ruled as strain. Employee had an MRI on 09/10/2016. Results were sent to us on 9/14/2016 at 3:36pm and degenerative changes found with cyst and small partial tear/fray with Degenerative Join Disease, and Osteoarthritis. Filing because of changes found that make it a recordable if not denied.
Working from a platform while installing concrete forms, employee slipped while picking up EE's pliers causing EE to hit EE's forehead on installed rebar. Employee received 13 sutures to EE's forehead and returned to work. Employee was using fall protection at time of injury.
While ascending a ladder to the booster outlet valve the employee missed a rung and fell contacting his chin on the guard frame of the booster.
EE was standing at an electrical switch panel waiting to fill the water truck he was assigned to operate. According to his statement, a snake came out of the bushes with its mouth opened and he thought it was going to bite him. He jumped back and fell on some gravels resulting in bruising to his left shoulder, ribs and lower back.
The employee was carrying a core barrel, stepped on an uneven surface and twisted his right ankle resulting in a sprain. He worked four days of restricted duty.
