Employee was operating Euclid R40 and had to do some maintenance. He climbed down ladder, then as he was climbing back up ladder, he got to the top rung and his foot slipped. He grabbed himself with the grab bar with his right hand.

Showing all 4Employee was putting oil in loader when he lost his grip on the hold bar of his loader. He was able to grab on to the loader without falling, but in the process he dislocated his right forefinger.
Employee was on Secondary Crusher belt when a co-worker started up the plant without following procedures. A second co-worker noticed that the plant had been started up with the employee on the belt and immediately yelled to stop the plant. The employee suffered minor injuries, which resulted in time off from work.
EE was pushing/lifting a heavy stone. Legs went out from under him. He did a split and felt severe pain.
