IE was carrying a top belt roller when EE's foot got stuck in the mud. IE then fell and landed on the roller hurting EE's back.
IE tripped over a block/debris, fell, and hit EE's head jamming EE's neck.

Employee jumped off the pod duster and felt a pain in right hip/leg.
Carrying a 72 inch top roller, tripped on a concrete block, and the roller smashed IE's thumb against the belt structure trailer.
Employee was walking backwards from the forklift after putting a chain on the tugger and tripped over a crib tie. EE landed on the right hand/wrist and ended up with a fractured wrist.
Pulling on a hose in the mud, slipped, and fell landing on shoulder.

On 11/13/20 employee slipped on the stairs in the prep plant and went to catch self hurting shoulder. Employee never a missed a day of work until 7/8/2021 when employee had shoulder surgery.
EE was getting out of the Security vehicle and felt something give in EE's right hip area.

Fuelman handed the dozer operator the fuel hose atop dozer. When dozer operator handed the fuel hose back to the fuelman, the fuelman took a couple of steps backwards when he slipped and fell to the ground.
Employee fell from ladder, he was wearing a harness and he suffered whiplash from the sudden stop. The ladder was slippery from an oil type material leaking from the top.
While installing belt clamps on belt conveyor, employee slipped and landed on his ankle. Weather conditions were slightly wet and could have been a contributing factor to the slip. He broke his right ankle.

A drill basket was being picked up by loader to move off door. Bucket slid off fork lift, employee tripped and fell underneath fork. Right knee caught underneath forks, causing strain and swelling.
