IE slipped while descending some steps in the prep plant. IE did not maintain three points of contact while on the stairs. Injury was not the result of a training or compliance issue.
EE was operating the headgate shearer when a rock came off the drum, hit the handrail then hit EE's leg. This caused the EE to fall on EE's side into the base lift jack. The EE sustained a cut to back of EE's left leg that required 6 sutures and the fall resulted in 2 fractured ribs. Rock was 1'x1'x6"thick.
While walking out of the Aleppo portal locker room, employee slipped and felt a pain in EE's right knee. Employee was diagnosed with a torn Meniscus and subsequently taken off of work.
Employee was using the fuel truck to grease the 815F compactor. EE was attempting to reel back up the grease hose when EE took a step backwards and fell. When EE fell EE's tailbone/back landed on the front blade of the compactor.
A Bailey Prep Plant grader operator was exiting the grader when the wind blew the door open and struck the employee. They fell onto the blade of the grader and hit their tailbone. They felt pain in the tailbone area. They finished their shift with the crew.
Employee got EE's foot caught and twisted EE's knee while repairing a sump jack on a Continuous Miner. EE did not feel immediate pain but began to experience pain when EE exited EE's mantrip at the end of EE's shift. EE worked all shifts until being taken off and scheduled for surgery to repair a torn meniscus on 5/5/2026
Employee was operating an excavator on a hillside. The employee stepped out of the equipment and when they stepped onto the ground they slipped on ice and fell on left shoulder. Incident occurred on 01-06-2026, however, initial doctors assessment did not show any injuries or restrictions. Follow up assessment was conducted today 02-03-2026 and a scan showed a torn muscle.
Employee was stepping onto a general car when they slipped. This caused them to strike their left leg on the general car. The laceration was treated with 1 stitch. The employee returned to work with no lost work days.
Employee tripped over loose material in a crosscut. Injury was not the result of a training or compliance issue.
Employee was observing the removal of a joint of 3" hose. When the staple was removed from one end of the hose, the hose blew out under pressure. The stream of water forced the employee to the ground, where EE injured EE's arm and hip.
Walking down stairs from the 5th floor in 1A to the 4th floor 1A. Felt the knee twist as walking down the steps.
The employee's pant leg got caught up in a fallen down branch while walking a stream for environmental monitoring. Upon trying to free EE's pant leg from the branch, EE lost EE's balance, fell and cut EE's left palm on a rock, requiring five sutures.
Employee was getting out of the shuttle car and stepped into a tire rut. EE twisted EE's right leg as EE fell to the ground. EE felt a pain in EE's right knee. EE continued to work all scheduled shift until being taken off of work to repair a torn ACL ligament in EE's right knee on 10/13/2025
Employee tripped while walking on the 4K longwall face. EE tried to catch self as EE fell and felt a pain in EE's left arm. EE continued to work all scheduled shifts until seeking medical treatment on 10/14/25 to treat a torn bicep muscle. EE began missing time on 10-14-2025
While employee was walking to the end of the track. They tripped and felt a pain in their right ankle. They had a follow up appointment on 9/9/25 and was diagnosed with a sprained ankle and subsequently taken off work at that time. Lost workdays began 9/9/25.
While the employee was hanging cables, EE slipped and fell backwards twisting EE's right ankle. EE worked all scheduled shifts until EE was treated for a sprained ankle on 8/21/25 and subsequently taken of work. Lost work days began 8/21.
A Bailey Prep Plant employee was in the process of installing new CMI belts. As EE stood on a working platform, the anchorage points of the platform failed allowing the employee to fall down the A1, A2 lean coal chute. In the process of falling down the employee struck EE's right knee against the opposing platform, causing a laceration below EE's right knee cap.
Employee was operating the headgate side of the longwall shearer. As they were cutting to the tailgate, the employee tripped. They struck their right calf off of the toes of the shields when they landed on the ground.
Employee was walking on uneven bottoms in the track entry of the G6 HG mining section and twisted a knee. Employee was diagnosed with a right medial meniscus tear on 06/24/2025.
Employee was attempting to get out of the skid steer. When EE stepped out EE's right boot got caught on the bucket causing EE to fall to the ground and land on EE's ankle
Employee was building a stopping, and was tackled to the ground by an employee of a different contracting company, resulting in a shoulder injury. EE continued to work unrestricted until 4/5/2025. Injury was not the result of a training or compliance issue.
Employee was hosing out a chute on slope belt and there was a pressure change in the hose. Employee got knocked off balance and accidentally tripped over some belt structure and landed on the ground causing injury to hip.
At #6 EE was parked beside the refuse shop. EE exited EE's pickup truck to take supplies to a truck driver that was in line at the bin. When walking back to EE's truck EE slipped on the ice and fell backward onto back. When EE fell EE's hardhat came off and ended up under EE's top of back/neck area. Employee did not miss work until 2/23/2025.
IE was using a tag line for a suspended load when EE stumbled over a traffic cone causing EE to fall to the ground injuring left ribs and elbow. After medical evaluation, IE had no broken bones but did receive one suture in the left elbow. 2 days later IE was having issues with pain, went to ER and confirmed 3 broken ribs.
Employee was walking to the parts car in 9 South Mains, when EE tripped causing EE's left knee to strike against a loose rock. Diagnosed with a torn meniscus. LTA as of 11/20/24.
Employee was walking along 3 Mother Belt when EE slipped and caught self before falling with EE's left arm grabbing the belt bed rail and felt a strain in EE's left bicep. On 9/4/24, MRI results determined employee has a torn labrum, and bicep tendon.
Employee was fire bossing slope belt 50' outby tail slope when employee lost EE's footing causing EE's left knee to bend back and underneath self while falling. The employee suffered a knee sprain and tear of EE's quadricep tendon. No other part 50.20-5 apply.
EE was walking on tight side of belt at 7 wall 2K long wall and went to step over coal accumulation and a rock that was about 2ft. wide and as EE was slipping EE caught EE's left foot under the rock and twisted EE's left knee.
Employee stepped through a man door and twisted EE's right knee. Employee was diagnosed with a fracture to EE's patella and was taken off work for rehab.
Employee was walking in 9 South Mains, when EE's right foot tripped on the mine floor causing EE to fall against the rib. EE was diagnosed with a torn Achilles tendon and fracture to the fifth metatarsal in the right foot.
Employee was carrying a crib bag on EE shoulder and, when they went to drop it, it caught on their mine belt twisting EE left knee. This incident did not involve any aspect of compliance with rules and regulations; mine equipment or mining system; job skills and miner proficiency, training and attitude; or protective items relating to clothing, or protective devices on equipment.
Employee tripped on uneven bottom and fell to the ground catching self with left hand on a sharp rock. This caused a laceration on EE's left hand requiring sutures to close. Employee did not miss scheduled work time due to this injury.
Employee was carrying and spotting J-Crib bags in the intake, when EE lost their balance and struck the right knee off of the ground. This incident did not involve any aspect of compliance with rules and regulations; mine equipment or mining system; job skills and miner proficiency, training and attitude; or protective items relating to clothing, or protective devices on equipment.
Employee was walking #4 Mother Belt conducting a pre-shift examination when they slipped on mud. From this event they fell to the ground and while falling EE attempted to brace themselves against the ground with right arm, which caused right shoulder to be injured from this action. Diagnosed with a torn rotator cuff on 12/5/23.
While walking to re-hook a chain onto a track rail that was being removed from 11L longwall, the employee tripped and fell onto the track rail. EE's knee landed on the on the track rail. Employee worked all required shifts until being taken off of work for a fractured kneecap on 9/11/2023.
Employee tripped on water hose while washing the floor in the foreman's locker room. They landed on their right side and experienced a cut on knee and bruise on elbow. Employee stated that whole right side of body hurt.
Employee was walking thru a crosscut, tripped and accidentally struck hand against a plate of a cable bolt that was on the ground causing injury to hand.
Employee threw a track switch. As EE was backing up to a manhole to clear the track to allow a mantrip to pass, employee stepped on a loose piece of coal and fell, twisting right ankle.
Employee was working on the surface area of an underground mine when employee tripped over a 2" hose and fell. This incident did not involve any aspect of compliance with rules and regulations; mine equipment or mining system; job skills and miner proficiency, training and attitude; or protective items relating to clothing, or protective devices on equipment.
Employee was rib bolting on the miner in the 3B section when they went outby to get a piece of wire mesh. As they pulled the mesh inby, they tripped on a rock and sprained their left ankle.
Injured employee got out of cab of scoop and walked toward the bucket end. IE stepped in a hole and tripped, landing on their right arm, dislocating right shoulder. Injury was not the result of a training or compliance issue.
Employee tripped and fell while walking past the loadcenter in 2K section. Employee used right arm to catch self and felt a pain in right elbow. Employee was diagnosed with a fractured Distal Humorous (small bone in elbow)
While walking outby from the face past the loading machine, employee tripped on the cables and fell to the ground, landing on right knee and shoulder.
Employee was walking to throw the 2 East Mains switch, while employee was walking the employee rolled an ankle. Resulting in an avulsion fracture. It didn't involve job skills, proficiency, training, or attitude. No PPE issues involved. No other part 50 criteria applies.
Employee was walking out of the 12J section, left foot stepped in a shuttle car rut causing employee to slip and fall. Employee felt pain in left ankle. Employee was subsequently diagnosed with a fractured bone in the ankle.
Employee was traveling along a belt drive and stepped on a roller causing them to slip and fall onto the roller causing injury to an arm.
Employee was side bolting in the 12J Section, as EE was retrieving 6 foot bolts from on top of the rear of the miner EE's right foot slipped off the step. EE's foot got caught in the miner cable and EE felt a pain in EE's right ankle. EE continued to work all EE's scheduled shifts until EE began missing work on 2/25/2022.
Employee was walking around the tail of the slope and EE stepped wrong and rolled ankle causing injury.
While hosing in the raw coal transfer building, employee slipped on wet floor and struck leg against a kick plate, causing a laceration. Injury was not the result of a training or compliance issue.
Employee was walking down the pan line in 8L Longwall Section, when EE slipped. EE felt a pain in EE's left shoulder when EE caught self. Employee reported that the pain has came back over the last couple of weeks. EE worked all scheduled shifts between October 10th 2021 until February 17th 2022 when EE was taken off of work due to the injury to EE's left shoulder.
