EMPLOYEE WAS LOADING SUPPLIES ON BOLTER. EMPLOYEE WAS CARRYING BOLTS WHEN EE TRIPPED OVER TOE GUARD, LANDING ON GROUND FACE FIRST. EMPLOYEE WAS IN THE INTERSECTION OF #8 ENTRY. THE AREA WAS APPROXIMATELY 60" HIGH AND DAMP.
Employee was stepping off of crane cab, slipped on steps and grabbed handrail, twisted knee as they slipped. Steps of crane were slightly wet due to condensation.
The employee was walking on the off side of the #6 belt when the employee tripped over a bottom irregularity (busted piece of bottom rock) that was sticking up approximately 8 inches high. There was 60 inches of clearance between the belt line and the rib. The floor was dry and relatively smooth with a mine height of 65 inches. The employee broke right radius bone (wrist)
Employee was crossing an overcast and EE's pant leg caught on the overcast causing the employee to lose balance and land on their right knee. The Employee has a bruise on the right knee. Employee is currently off of work at this time and employees return is unknown at this time.
Employee was walking through the crosscut between the power entry and the belt entry when EE slipped causing EE to fall resulting in a fracture to EE's fibula in EE's left leg. The employee is currently off of work.
Employee was unplugging a set of scoop batteries when employee lost balance and hit lower back on the corner of another set of batteries resulting in employee breaking transverse process. The employee is currently off work and actual return date is unknown at this time.
Employee was moving cable near the roof bolting machine when foot became lodged and as EE turned, EE twisted right knee. Employee is currently off work waiting on an MRI.
Employee slipped near the #7 belt head rock box and twisted EE's left ankle. The employee was taken to Raleigh general hospital where an x-ray showed EE's ankle was not broken. The employee then had a follow up appointment on 2-8-17 where a second x-ray showed that the ankle was broken. Employee is currently off of work and EE's return to work is unknown at this time.
Employee was cutting the belt with a gas chop saw. EE was standing on the belt when EE made a final cut through. The belt moved and employee fell; striking elbow on the roller frame. Supervisor was notified and first aid was implemented and no other treatment was necessary at that time. On 07/15/16 employee presented with elbow swelling, pain and discomfort.
Employee was climbing the steps on an endloader when the bottom step broke causing the employee to slide down the ladder landing on EE's feet jarring EE's back. Note: Employee came back to work and worked until 3-17-16 at which EE was placed off of work due to a medical evaluation.
Employee was exiting a roof bolting machine when employee tripped and fell on employee's hand dislocating employee's left pinky finger.
Employee was working on ventilation when his leg got tangled up in a piece of line curtain causing him to fall hurting his right knee. Note: employee returned back to work on 10-21-15 and worked until 2-29-16. On 2-29-16 he was placed off of work due to a medical evaluation.
Employee was hooking up a down drill near a section tailpiece and slipped and fell to mine floor landing on his left wrist. Caused a hairline fracture to his left wrist.
Employee was entering D8T dozer, as he turned his footing slipped and fell on his right wrist. Right wrist was fractured.
Employee was trying to straighten a bolt when his hand slipped off the bolt and employee's body fell forward and he caught himself with left hand causing a sprain to his left wrist. Received a wrist wrap from doctor and returned to his regular duties the following shift.
Employee was walking from service hoist to bath house and slipped and fell on his SCSR that was attached to his belt.
Injured removed a piece of grating at the surge bin area where coal was fed onto slope belt. He did this to drop a pump through hole by using a hoist. The slope belt shut down but the vibrator did not completely stop. When injured attempted to shut vibrator off he forgot about the hole and fell through to lower area.
Employee was working on belt drive and slipped and landed on his tailbone.
Employee was getting out of forklift when his foot got hung and as he was leaning forward to catch himself the incident occurred. Broke bone in his left wrist.
Employee stepped off the diesel mantrip into 4-5" of water near spad 260 at the elevator and twisted his right ankle. This is to report medical treatment in the form of orthopedic immobilization. (Crutches/Cane and Ankle Splint)
Fireboss opened a mandoor and stepped through onto uneven bottom and his weight was posted on his left knee twisting it. Initial x-ray report showed no injury but he was told not to work until he got an MRI that took 12 days to get and 3 more days to get the results. Notified on 6/3/13 that he had sustained an ACL strain with cartilage damage and needed to remain off work.
Employee was on ladder removing tin off hoist house walls. Gust of wind blew down house, employee fell off ladder injuring Right ankle and left wrist.
Shuttle car operator parked his shuttle car and when he stepped out onto uneven bottom he twisted his left knee.
Employee was traveling inby on the slope. He lost his footing and began to fall down and as he fell he struck his right hand on the belt and dislocated his ring finger at the middle joint. This is to report medical care only no work days were lost.
Employee was operating the #10 shuttle car, as he stepped out of the car to turn his body around he twisted his knee. Mine management was not aware of this incident until 07/31/2012.
Employee stated he was hanging flypads and turned towards his scoop and felt pain in his right leg. The company believes this injury relates to an injury that occurred on 6/9/12 that the company does not believe occurred at the mine.
Employee stated he was operating his shuttle car and when he got out to turn around so he could change directions he stepped on a ledge twisting his right knee. He was diagnosed with a sprain/strain and given a knee brace. This is to report medical treatment only.
Roof bolt operator was tramming roof bolt machine to a new place and the injured employee was walking approximately 25 feet behind the machine to spot for the roof bolt operator to ensure he did not tram into the cables on the off side of the machine. The injured employee tripped over the roof bolt cable and fell to the ground injuring his right knee.
