EE dismounted water truck and walked towards water stand. Feet got stuck in mud and ee fell forward to the ground, catching self with both hands. Seen at ER where right wrist fracture was diagnosed and the ee was put off work pending consultation with an orthopedist.

EE was walking ribbons during metal removal. EE states was walking a ribbon and stepped down funny because the ribbons are not level, EE felt pain in the knee. EE was seen at the ER and diagnosed with a strain to the knee and put off work.

The employee stepped off of a concrete step and twisted their right knee.

Ee was doing a final walk around of equipment at the end of shift and twisted ankle. EE was seen at the clinic the evening of the 10th and put off work pending follow-up. EE was continued off work after follow-up and diagnosis of a severe ankle strain.

Ee was dismounting grader to continue pre-shift inspection. EE lost grip with right hand, lost balance, and fell to ground from 3rd step on ladder. Seen at ER where ee was transported for surgery on an open fracture to right ankle.

Ee was dismounting haul truck and "hopped" down from the last step, landing on right foot. When ee landed, ee felt pain in foot. Ee worked next night shift and went to the doctor the following morning. Ee was diagnosed with a fractured bone in right foot. Released with restrictions that site could not accommodate.

Ee was climbing a ladder to remove a trip cable at 351 shovel located at pipeline. Bango arm swung around and struck the ladder, causing the ee to fall approximately 5 feet to the ground. Ee landed on left side. Fractured left arm, bruised left knee, strained left ankle. Off work pending an appointment with orthopedic surgeon.

EE was pushing wheelbarrow through mud and it tipped over. EE went to brace themselves and landed on their right thumb. Fractured right thumb. EE released to restricted duty which the site could accommodate.

Contract employee was taking a mop bucket out to the janitor closet outside of the dry. EE slipped backward on the wet floor and fell, and was struck in the face with the bucket. EE was transported by light vehicle to NNRH with a cut over the eye. Subsequent follow-up at University of Utah resulted in stitches and 3 days off.

EE slipped and fell on EE's left elbow while doing a pre-op inspection. EE was seen at the ER and diagnosed with a contusion, follow up resulted in EE being put off work.

The employee lost his balance while reaching for a small rubber mat and felt pain in his left Knee.

While rigging the discharge hose to the top-head the employee slipped while re-positioning himself. The employee stuck out his hand and pinched his little finger between his palm and the top-head. The resulting wound required three stitches to close. The employee was wearing his gloves.

Ee was doing a preop inspection of forklift. She stepped off forklift into parking ditch, rolling ankle, and landing on right wrist. She was seen by a doctor and put off work for 3 days.

Ee was discussing field level risk assessment with other ees. He turned and twisted with right leg planted and felt a pop and immediate pain. When pain did not subside, he requested evaluation by a doctor. Diagnosis of torn meniscus by orthopedist on 01/15/15 after MRI review. Surgery scheduled 02/04/15.

Ee rolled his ankle after stepping off a skid steer. He was seen at the ER, referred to a foot and ankle specialist, and was diagnosed there with a fractured ankle. Ee put off work.

Ee was working with shovel cable, the cable pulled tight and the ee was knocked off his feet. He hit his head on the ground, cutting his forehead. Seen at the ER, stitches were required to close the wound and the employee was released to full duty.

Ee had finished changing a transmission filter on a piece of equipment and was stepping down to the step to dismount when he slipped on the step and landed hard on the ground on his left ankle. There was mud on his boots and the step was damp. He was seen at the ER and diagnosed with a fractured left ankle and put off work.

Ee was walking and stepped over a parking ditch and heard a pop in his left calf. He had pain. He was evaluated at the clinic and placed on restricted duty and the leg was wrapped. No restricted duty available, so the employee lost 3 days. Released to full duty 06/25/14.

Ee got off drill, walking on level ground and stepped/fell in a previously drilled hole, straining his ankle. Seen by a doctor, diagnosed with a strain, given crutches and restrictions that could not be accommodated. The ee had previously scheduled vacation so no lost or restricted days were incurred.

Ee was working on the blast pattern and she slipped on wet ground. She caught herself and didn't fall, but felt immediate pain in her right knee. She was able to return to her truck, but unable to bear weight after that. Seen at clinic and put off work pending an appointment with an orthopedist. Diagnosed with a torn hamstring and given physical therapy.

Employee was doing maintenance on the fuel hose reel when he was climbing down from the truck his left foot caught on the tailgate cable and he fell 3 feet to the ground resulting in radial fracture to left arm.

Ee was doing a post trip inspection of the track dozer. He slipped on an icy spot on the dump and fell, injuring his ankle. He sought treatment on 12/05/13 and was referred to an orthopedist. The orthopedist placed him off duty and referred him to a foot and ankle specialist.

Employee stepped off RTD into a hole and twisted his left knee. He felt a pop. He was taken to the ER and diagnosed with a ruptured patella tendon. He was given restricted duty pending follow-up with an orthopedist. He was seen by the orthopedist and requires surgery for tendon repair. Surgery 04/10/13. First lost work day will be 04/11/13.

Employee slipped on ice while walking between two drills. Seen at ER, diagnosed with a fractured tailbone and released to full duty. Unable to return to work due to pain.

There was snow on the road to disposal site. The air mover lost traction at top of a hill along the road & was unable to continue to disposal site. There was ice on the ground where the air mover was disabled. EE exited vehicle to set wheel chocks. He was wearing required footwear, but no ice cleats. He sustained a fracture to right elbow, but is released to work w/restrictions.

While walking to the blue room, the employee stepped on a rock, twisted his ankle and fell to the ground. Seen at ER, restricted duty for strain ankle.

When stepping down off steps, rug slipped and the employee hyper-extended her right knee. She was seen at the ER and referred to an orthopedist. Diagnosed with a sprain/strain and put on restricted duty. Unable to work due to medication needs.

While unlocking a 4" hose from a truck the hose was still under pressure even though the valves were shut off and was unable to bleed line off due to tank being full and no bleed off valve on fill station. Hose kicked back while EE was holding it causing him to get pushed into Jersey barrier and during process chipped/broke a tooth.

After participating in a daily toolbox and safety meeting, the employee left the line-out office and was walking to the equipment ready-line. The employee slipped and fell on the snow covered surface resulting in a knee injury.
