Employee exited the mantrip and started walking to the face when EE stepped off of the concrete surface onto the mine floor and lost balance causing right leg to be caught in under EE's body fracturing the right ankle.

Showing all 5Employee was stepping out of skid steer on July 3 and caused injury to knee but did not want to seek medical treatment. On July 9, 2018, employee decided to receive medical treatment for incident and was placed on light duty.
Injured was attempting to drag a piece of shuttle car cable across the section tailpiece when his foot slipped on loose material causing him to strike his left upper thigh against the belt structure. This resulted in an approximately two inch laceration. He was treated at the mine and taken to the hospital where the wound was closed with nine stitches
The employees was walking the belt entry into the mine at 17 break on the number 4 conveyor belt when he slipped and fell to the mine floor striking his knee and right shoulder.
Walking down the haul road and fell and bruised eye. Employee stated he had an episode due to his medication.
