Showing all 5Miner slipped from the 2nd rung of 3 foot ladder when EE lost grip while tightening a bolt on a pump. EE fell to the concrete floor and suffered bruising to lower back and hip. Ladder was dry but 3 points of contact had not been maintained.
Employee was descending a slope into a dry pond when EE felt a pop in left leg, suffered a ground level fall, and noticed leg would not support EE's weight when EE tried to get up. EE was taken in for evaluation and returned to work for the next normal shift by the Physician. Worked until surgery on 10/17/2019.
