Showing all 5EE was blow piping down the ore chute. EE was wearing fall protection at this time. EE finished blow piping, sat it down on the walk way, took fall harness off and took a step forward. A metal wire poked EE in the face, EE was startled jumped back. EE fell down the ore chute, 30 feet below and landed in the 7 ton young buggy.
While moving cabinet down stairs, one miner missed bottom step injured miner adjusted footing missed step and fractured ankle.
The miner was attempting to board an electric Loci that he had parked under an 8" air line. When he stepped on the wet step his foot slipped causing him to hit the right side of his head against the air line. He did not report this till the end of shift. He declined first aid or medical attention.
Employee slipped on icy switch and tore calf muscle.
