Employee tripped landed on EE's hand sustaining a fracture.

While working in the bucket elevator the employee stepped down on to a beam, causing the employee to lose balance. Employee reached with left arm which dislocated employee's shoulder.
Employee was ascending stairs, tripped on a step, and stuck hand out and it went through glass light. Employee sustained a laceration to the right index finger requiring 6 stitches.
Employee rolled ankle on mine floor was given a full medical release on 09/04/2023. On 10/10/2023 went to see an orthopedic and was put off work for two weeks.

EE was pulling on a hose while cleaning a building and tripped and fell against a motor. Initially injury was not determined reportable due to only requiring first aid treatment, further medical review revealed that the EE will need surgery to affected left shoulder injury. Determined reportable on 8/13/2015.
