The employee was rock dusting in 4 North Seals Airway, when a hole developed in the rock dust hose. The injured was working their way out to the track in the dust, when they tripped and fell through a mandoor injuring lower back.
EE was walking down some o/c steps at 15 D #2 seal. EE's right knee turned and caught self on the handrail of the steps. This action twisted EE's right hip and lower back causing pain. The employee finished EE's shift.
The employee was working on 1WHG L/W Set-Up Face. While setting a shield, the EE climbed over the pan to pull slack for the stinger. When EE stepped back over the pan, EE's left foot slipped and the EE twisted left knee.
The EE was stepping out of D700 motor at 78 block St. Leo Haulage, when EE's left foot hit the ground EE's ankle rolled.
The employee was walking down the #3 heading in the 1WTG Section. The employee's foot got stuck in mud, and when EE tried to step out EE's foot slipped on the dry pavement injuring right leg. No laws or company polices were violated, and was wearing all the appropriate PPE.
The employee was working on the first floor of the plant, helping remove a pump. EE was steadying the pump by holding a chain, when EE lost grip and slipped on the floor. EE's lower back and tailbone struck the floor injuring EE. EE was wearing all appropriate PPE and did not violate any laws or company polices.
Employee was climbing out of supply motor and while doing so felt a pull on left side hip. Employee was wearing all required PPE.
After drilling a hole employee placed a PVC Collar pipe into the hole. While pushing the PVC pipe into the hole with EE's left leg EE felt a cramp causing EE to fall. Employees leg was dislocated and later found to be fractured.
The employee was exiting an end loader in Sugar Run Yard, when the employee stepped down the employee's right knee twisted. The employee was properly trained, and did not violate any laws or company polices.
The mine received a Self Insured Employer Decision on 6/21/2021 from a medical report dated 6/9/21 awarding a 4% permanent partial disability. This is from an incident from 8/3/19, where the injured tripped while walking down steps in the preparation plant.
The employee was on the 1 west tailgate section, helping others pull a cable past the DMU/Belt. The employee got right leg caught in the slack of the cable, and fell to the mine floor twisting right knee. The employee was complying with all laws, company policies, and was wearing all appropriate PPE.
The employee was stopped at 12 block 9 South Haulage on 86 mantrip. When the employee stepped off the mantrip, EE landed on a rock. EE twisted EE's right knee and fell to the mine floor. The employee was following all laws and company policies, and was wearing all appropriate PPE.

While the employee was walking near a stopping on the 6 East L/W Section, the employee slipped and fell on right side injuring back and ribs. The employee was trained, did not violate any company policies or laws, and was wearing all the appropriate PPE.
The employee was on a step ladder plastering a stopping. The employee slipped off the ladder and landed on a block, injuring upper back, right shoulder, and neck area. The employee was trained in the task and was wearing all the appropriate PPE. The employee did not violate and laws or company policies.
The employee was carrying one base lift jack on EE's shoulder to deliver it to the face of the longwall set up and tripped over a crib. The employee fell down and the jack landed on EE's right lower arm causing a contusion. The employee did not violate any laws or company policies.
The EE was straightening up materials on a supply car that was parked in the Crash Pit. The employee went to step back and slipped, striking EE's right shin on the supply car. The employee had some swelling and a contusion. The employee did not violate any laws or company polices and was wearing the appropriate PPE. The employee worked up to 1/19/19 when EE began missing work.
Contract employee was exiting vehicle with mud on soles of boots when the contract employee slipped on the step, landing awkwardly on the ground, spraining ankle. Cause of this injury was employee awareness and muddy conditions. Compliance with rules/regs, PPE, mining systems was not a factor, Mine proficiency may be a factor
The employee was walking by the supply cars in 7 East Section, when EE tripped fell and cut hand on a track tie laying up against the rib. The employee received five stitches.
The employee was walking down the steps at the Flo-Dryer, when at the last step before the second landing tripped strained leg and struck right thigh on the hand rail.
The employee left the control room of the plant, while walking through the plant the employee stepped on a floor drain. The drain cover came off, employee's foot went in the drain. The employee twisted left knee & struck it on the plant floor. Upon going to the doctor on 11/20/18 employee received a compression sleeve for left knee & will be scheduled to physical therapy.

The EE was walking through a spur to spot a roller on the belt line. The EE tripped over a roll of chain link fence and fell on the roller the EE was carrying. The employee was wearing all the appropriate PPE and did not violate any laws or company policies.
Rear end of the continuous miner had just advanced inby the #10 crosscut of the #3 entry of 6East. The injured went to pick up the miner cable at the inby corner of the crosscut (so that it could be cleaned with the loading machine) and stepped on a loose rock, rolling their right ankle and resulting in a sprain/strain.
While walking along the walkway of the Fletcher Walkthrough Crawler CDDR-17 dual head roof bolter in the St Leo section the injured miner rolled ankle and fell down, resulting in a dislocated ankle and foot.
Carrying rib bolt supplies past 3rd car from end of track (5E #2 entry) and stepped in a rut rolling left ankle and resulting in a sprain to the soft tissue of left ankle.
Operator was cleaning out crosscut from track to 3 belt on 1 north at 15 block. Injured was pushing wheel barrow of material that tilted causing ee to fall down and strike knee on the mine floor, resulting in a contusion.
Injured was installing side boards on #10 Shuttle Car in the St Leo working section. EE was in the bed of the shuttle car and when attempting to dismount the shuttle car (to go and get parts) EE twisted left knee when stepping down from the bed of the shuttle car down to the mine floor, resulting in a strain/sprain.
Accident report not filled out with the mine operator until 12/8/17. Injured was taking an air drill from 64 to 59 1/2 block on walk side of 11 belt when EE tripped, fell down and rolled. EE injured EE's shoulder in the fall, being diagnosed with a pinched brachial nerve in EE's right shoulder area.
Employee was getting out of ee personal truck in the Sugar Run parking lot upon arriving to work and twisted left ankle when stepping onto the ground resulting in a strain to the soft tissue of left ankle.
Injured was pulling cables from the 1E HG longwall power car during a power move and slipped while stepping over cables already on the ground and over extending the left knee. Employee had returned to work on 10/2/17 from a previous injury to this same knee that was sustained at home. Referred back to previous orthopedic specialist for evaluation.
Getting off 604 scoop to unload supply cars in the St Leo supply area and the employee tripped getting out of the operators compartment and landed on their left wrist, resulting in a strain/sprain type injury.
Injured was carrying a top roller (removing it from the walkway) just inby 59 block 1 East Head Gate belt (track side of belt line) and tripped, falling down and receiving multiple injuries. The most severe was a fracture to sternum and a fracture to the L1 vertebrate.
Injured was walking to the pumper switch on Miracle Run bottom and stepped into a bottom irregularity with left foot and twisted left knee. Reported to work on 8/12/17, but started missing work on 8/14/17 due to pain and swelling in left knee.
Injured was walking back from 4E section power car after putting power back on the section auxiliary fan and attempted to step over some ventilation tubes. EE tripped and fell, landing on right elbow. EE continued to work until 7/1/17, when EE went off work pending surgery to repair a torn tricep tendon in right elbow.

Going through stopping located at 306-307 in the 9 south #2 entry and air pressure closed the door on EE's foot. When EE got foot out, EE fell down and stoved wrist on the floor trying to catch self, resulting in a possible fracture. No problems with mining methods, ppe, training or compliance issues.
Employee was walking along supply cars at the end of the section track and tripped on a piece of track rail causing employee to fall. When employee fell, struck right knee on the rail. No issues with equipment, PPE or training.
Walking down 2 track Metz bottom and tripped over kicker tie (tie installed to slew track away from rib) and fell, landing on the light battery which was pushed into left hip and caused pain in hip and groin area. No problems with mining machines, PPE, training or compliance issues.
While walking toward the back of the crash pit (slope bottom) between most outby car and handrail EE slipped and fell, grabbing handrail on descent and causing pain in right shoulder area. Subsequent medical attention put EE off work until after a MRI of right shoulder. No problems with mining methods, training, PPE, or compliance issues.
Employee picked up trash on second floor of dump and leaned over handrail to throw the trash in a coal car in the crash pit, the rail gave way and ee fell to the crash pit floor receiving a contusion to left arm and leg. No problems with mining methods, PPE, or training.
Employee was walking down 14 belt to examine a solcenic leak and slipped on some mud and hit right forearm against the belt structure when ee fell, receiving a contusion. Became reportable when started missing work on 7/12/16. No problems with mining methods, PPE, training or compliance issues.
Carrying roll of curtain down 2E track entry and stepped on solcenic hose by longwall pump car and fell, twisting left knee and injuring the soft tissue of the knee. Diagnosed with strain/sprain. No problems with PPE, training, mining methods or compliance issues.
