Preparing to move screen plant when employee stepped on berm and felt right knee pop. Continued to walk around and knee popped a few more times and was sore. Became reportable on 9/12/25 when employee had surgery for medial meniscus tear.

Showing all 4Employee had loaded a Sandvik Ranger 800 drill onto the lowboy to move locations. The employee crawled up on the lowboy then the drill to add fuel. As EE was fueling EE slipped off the track of the drill and fell approx. 5 feet to the ground. EE as taken to the ER and found nothing to be broken. EE was not put on any restrictions but was given something for the pain at er.
Employee was removing a piece of belting from Belt #11 when they slipped forward striking the frame with their left side. Employee did not fall to the ground. The employee did not wish to seek medical evaluation. On 1/23/23 the employee was still sore and went for evaluation where an x-ray revealed two fractured ribs and bruising. Employee was placed on restricted duty.
Employee was exiting the skid steer, company #23.600174 when EE slipped and fell. Employee declined medical care. On 2/18/2021 employee went to occupational clinic where EE had an x-ray which was positive for a fractured left rib making this a reportable injury.
