The technician was removing a broken hand rail from the right side of a 777D. EE had the handrail off and turned to go down the ladder to hook to the new handrail to install. EE unhooked the fall restraint to come down the ladder. EE's left foot slipped due to mud and rain. EE did have three points of contact but the slip pulled EE and EE fell to the ground on their back.

Showing all 6Employee was walking from the 3rd floor to the 2nd floor. As employee placed foot on the 3rd step from the bottom, the step broke loose from the right side looking down causing employee to lose balance and fall to the concrete floor. Employee landed on right knee and arm.
Employee exited elevator and slipped and hit on EE's right shoulder. EE continues to work and started physical therapy on 7/14/2022.
Employee was changing out a bag of floc. Some of the floc spilled on the floor and employee stepped on it causing EE to slip and fall. This resulted in a sprain to EE's right knee. Employee stated physical therapy on 8/3/20.
EE was washing between two I-beams adjacent to the hose storage near the primary cyclones on the 4th floor of the plant. When EE finished, EE turned to store the hose, but EE's boot got caught between the beams. This caused EE to twist EE's knee and start to stumble. In an attempt to catch self EE states EE strained EE's lower back also.
Employee stepped down from water truck into a rut and turned right ankle resulting in a sprain.
