Employee was welding on the ground level, suddenly EE turned to the opposite side of work area and tripped over a piece of wood lying on the ground. EE fell placing hand on the edge of a structural component H-Bar 3" flange x 4" x 6" base, the sharp edge of the material caused a deep cut in EE's right hand. EE was wearing all the PPE and proper gloves.

When attempting to open difficult railcar out gate, employee applying pressure to bar lost EE's balance when the gate came loose. EE fell to the side, same level, rolling EE's right ankle.

EE lost footing while they bent over to pick up an extension cord in front of the main office building. The EE caught balance by placing their right hand on the rim of a metal drain cover. EE sustained a laceration to the web of the thumb requiring 6 stitches. EE was released with no restrictions.

Electrical contractor fell thought ceiling tile while running electrical wiring for new motor control panels. Miner did not use the fall protection harness and misjudged the stability of the ceiling tile to hold ee's weight. Miner was trained on fall protection use. Miner sustained fractures to head and left wrist from the fall.
