Employee was in route to clear an obstruction in a surge bin; while ascending a vertical ladder the employee lost their gripping and fell to the platform below. This fall was approximately 9 feet, and employee landed on right knee which caused a laceration to that knee. Said ladder is in good condition, with a safety gate at top landing, and has a proper back cage installed.

Showing all 2Employee was assisting the maintenance crew change bags on the bag house and it started to rain so they set the cover to the access hole back on top of the hole to wait out the rain, after doing so employee stepped on cover which was not secured. Upon doing so the cover tipped upward resulting in the employee's right foot falling into the access hole.
