Showing all 26IE was stepping over belt structure and when employee went to step over structure lost footing and felt a pop in right ankle. IE was treated conservatively and didn't miss any scheduled shift until 03/31/2026 when it became an LTI.
IW was shoveling belts and slipped. IE threw their shoulder backwards with shovel in it to catch themselves. When IE threw shoulder back, they heard a pop. IE was taken in for evaluation and when results came back, they suffered a rotator cuff tear on right shoulder.IE continued to work until 07/22/2025 and then went out for surgery.
IE was walking down Longwall face and tripped over relay bar landing on their hand and causing Carpel Tunnel in their wrist. IE continued worked through the carpel tunnel until June 27th, 2025, when surgery was required to fix the issue.
Employee was taking the top cover off the driver to work on the gear box. The EE was positioning themselves by the machine when they slipped on a hose, which was on the ground, and fell forward. They grabbed onto the guard of the machine, but continued to fall and contacted the ground causing a Closed Fracture to Upper Left Arm.
Employee exiting out of skid steer stepped between the bucket and mast. IE's foot got stuck, and IE went forward fracturing left ankle.
IE was doing hoist check on Elevator and was walking down stairs and slipped catching themselves. IE pulled their shoulder in the process. The incident took place on 02/20/2025 and employee never missed a regular scheduled shift until 08/04/2025. IE had shoulder surgery.
Employee walking down the stairs on NM #1 beltline. Employee slipped and fell landing on their left shoulder. IE continued to work normal shift until MRI was complete and determined EE would need surgery on their shoulder. IE didn't miss the first day of work until 01/17/2025 which is first day it became reportable lost time.
IE was walking around belt winder and tripped over hoses connected to belt winder. EE stumbled and when EE went to catch self came down on EE's right foot awkwardly and fractured EE's metatarsal on EE's right foot.
IE slipped and fell on EE's left shoulder while rock dusting on an overcast area. The incident took place on 4-14-24 and employee continued to work through EE's scheduled workdays. Employee received an MRI where EE suffered a torn tendon in EE's left shoulder on 5-1-24.
IE was getting into underground ride and slipped hitting IE's head on the top door frame jarring IE's neck causing a stinger down the shoulder blade. The incident occurred on 7-11-2023 and employee was still able to perform the normal work duties until 3-5-24 when employee wound up having surgery to fix the issue.
IE was pre-shiftng LW face when IE tripped walking off the LW face. Employee went to catch self with right arm falling on it and sustained a fractured right shoulder.
IE employee was walking the warehouse yard conducting an audit on supplies needed ordered. A 1/4-inch nylon rope was sticking out from underneath a pallet and IE snagged their foot on the rope causing EE to trip and fall. The initial accident was on April 18, 2023. June 2nd employee became a reportable.

IE was loading timbers on a ride when EE's foot slipped causing EE to lose balance while holding a timber. When EE tensed up to stop self from falling, EE felt a sharp pain in the back.

Employee was in the process of getting out of the cab of a mantrip when EE's right foot got caught up and resulted in them falling onto the ground onto a 6x6 timber.
Employee was climbing out of the shield and lost footing causing left knee to twist. Employee finished the shift and continued to work. EE was seen by a physician 4 times before requesting a second opinion. The second physician administered shots of medicine into the knee and wrote the employee off for 2 shifts. The first missed shift was 2/5/2020.
Employee was washing down on the slope belt when they twisted their left knee.
