Showing all 12IE was taking apart water line after a power move on the longwall. As the IE stepped under the monorail IE slipped and strained IE's back. IE did not seek medical attention until 5-13-20.
IE was carrying structure for a belt move. As the IE was walking back IE tripped over a hose that was in the walkway. As IE tripped IE fell into the side of the feeder bruising IE's ribs. IE did not request medical treatment until 5-8-20
IE was changing an underflow pump when IE stepped in a hole. The IE then fell and twisted knee. IE finished shift then decided to seek medical attention.

IE was taken to hospital for further evaluation. This incident was called into MSHA due to EE possibly going unconscious on the ride out. Dr's at hosp. didn't find anything wrong but EE's Dr. says possible concussion?
IE was getting out of the scoop IE was operating. As IE stepped out, IE lost IE's footing & fell. On the way down during IE's fall, IE smacked IE's upper lip on the door of the scoop IE had gotten out of. IE returned to work on IE's next shift & did not miss any work. IE received 2 stitches in IE's upper lip.
