employee was walking down the entry tripped on rocks in travelway, falling to ground catching catching self with hands
IE was taking apart water line after a power move on the longwall. As the IE stepped under the monorail IE slipped and strained IE's back. IE did not seek medical attention until 5-13-20.
IE was carrying structure for a belt move. As the IE was walking back IE tripped over a hose that was in the walkway. As IE tripped IE fell into the side of the feeder bruising IE's ribs. IE did not request medical treatment until 5-8-20
IE was changing an underflow pump when IE stepped in a hole. The IE then fell and twisted knee. IE finished shift then decided to seek medical attention.
Employee was climbing out of a lotrac and stepped on uneven ground. Employee then felt a pain in the right knee. Employee did not report the incident until 3-6-2020 and did not miss work until 3-10-2020.
Slipped on Main North 2 Belt while hand dusting. When EE fell EE landed on shoulder. EE came to work the next day and said EE was in pain. EE was then taken to the doctor.
IE had walked to the back of the motor pit underground. IE then turned to walk back and tripped over a block in the walkway. IE landed striking left shoulder against the track rail. IE was brought out and sent to the hospital for treatment.
IE was walking, EE then slipped and fell going down a step and injured EE's left leg. IE was seen by a doctor and is awaiting a follow up appointment.
IE was setting up to wash the walkway on the silo belt. IE had gotten hose ready to pull and when IE walked around the corner of the MCC building, IE tripped over the hose that was to be used to wash with and fell which caused a torn tendon in the ankle.
IE was doing a pre-operational check on the locomotive that EE was going to operate during EE's shift. As EE was stepping forward, EE twisted EE's right knee. The Dr. found no damage to the knee but took EE off of work for 1 week.
IE was going through a mandoor to grease the 20E drive motors. As EE went through the door, EE's foot hung causing EE to fall to the ground & hit EE's head. IE was taken to hospital for further evaluation. This incident was called into MSHA due to EE possibly going unconscious on the ride out. Dr's at hosp. didn't find anything wrong but EE's Dr. says possible concussion?
IE was getting out of the scoop IE was operating. As IE stepped out, IE lost IE's footing & fell. On the way down during IE's fall, IE smacked IE's upper lip on the door of the scoop IE had gotten out of. IE returned to work on IE's next shift & did not miss any work. IE received 2 stitches in IE's upper lip.
