Employee was standing at the Main North 3 Belt Drive area while spotting a splice. EE turned to walk away and EE's feet slipped out from under EE causing EE to fall. As EE was falling, EE's hard hat came off and EE struck head on a cradle. It did not involve any equipment or compliance issues. It also did not involve job skills, proficiency, training, PPE or protective devices.
