Employee was walking in the #5 entry of 3rd East Mains between XC 6-7 when foot slipped and EE fell to the ground. As EE fell to the ground, EE reached out with left hand to catch the fall, resulting in an injury to EE's left thumb. X-rays indicated a fractured thumb.

While walking up the longwall face at approximately shield # 112, the employee tripped and lost balance and hit head on the shield leg.

Employee unhooked a chain that was holding up a bundle of wire mesh roof screens that were leaning against the rib in the 12th north section. The bundle of screens fell over and hit the employee and knocked EE to the ground.

The employee was getting out of the cab of a shuttle car at crosscut 37 in the 4th East Mains Development section, when EE's foot slipped on the mine floor, causing EE to fall to the mine floor. As EE fell EE tried to catch EE's fall with EE's left hand, resulting in a fracture to EE's left wrist.

Employee was unloading 60" belt structure off of a flat bed belt structure trailer located at the mouth of the 12 North section. As employee stepped off of the trailer, employee sprained right ankle.

Employee was removing a Jennmar Metal stopping in the 9th North set-up face when EE's foot slipped on the damp floor resulting in the employee sustaining a right ankle sprain.

Employee was mudding a stopping at crosscut 12 in the 4th East Mains when EE slipped and fell. EE's left arm hit the pump skid resulting in a laceration which required stitches.

Employee was walking down stairs and reported EE tripped on the stairs and felt a pain in EE's back.

The employee was in the process of dropping a vent tube from the mine roof when the employee's foot slipped in the mud. As the employee fell to the ground, employee injured left lower leg. The result was a hairline fracture to left fibula.

Employee had just finished checking the shuttle car anchor and as they turned to walk away they slipped and lost balance resulting in spraining right ankle.

Employee was stacking metal roof mats onto the roof bolter machine. EE had lifted one end of 2 mats and placed it onto the machine and then walked to the other end of the mats and lifted them off of the ground and was sliding them onto the stack of mats, when EE slipped on the damp mine floor and jammed their right index finger.

While walking in the 9th North belt entry, the employee tripped over a belt tailpiece anchor pin and and landed on left knee resulting in knee damage that required surgery.
