Injured employee was carrying a belt roller with both hands along the 7 North #1 Belt between 24-25 blk. when they tripped on a piece of loose coal and lost balance. They fell against the side of a roller cradle, smashing right hand between the cradle and roller they were carrying. Suffered a contusion & swelling. Was wearing required PPE including metacarpal gloves.

Injured Employee was walking beside a parked Shuttle Car and slipped on loose coal (sloughage) and fell against the rib. They stuck right forearm out against the rib to catch themselves. Forearm hit a spad that was in the rib causing a small puncture and a cut requiring 3 stitches to close. Employee has training and experience. Returned to work next scheduled shift.

Injured employee was walking and tripped on a crib block causing EE to stumble and roll EE's right ankle. Has been diagnosed with a torn ligament. No rules or regulations were violated and employee has training and experience.

Employee was operating a diesel locomotive on the 8W CM Section track. They parked the motor and stepped out of the deck onto a steel tunnel liner for hoses to pass under the track rails. They slipped on loose gravel that was on the steel cover, slipped and fell down, landing on their right upper leg. No rules or regulations were violated & employee has training & experience.

Employee was stepping out of the diesel locomotive they had parked. They stepped off the locomotive as they reached for a handle for support but the handle wasn't on this locomotive. Lost balance and fell landing on right shoulder. Has muscle damage in the shoulder. Employee has training and experience. Was wearing required PPE.

Injured employee was hanging a check curtain on the 7 North Mains CM Section. EE used an 18" high work platform so EE could reach the mine roof. While standing on the platform, EE pulled up on the curtain, lost EE's balance and fell off it, landing on EE's right shoulder. Result was a dislocated shoulder. Employee has training and experience. No rules or regulations were violated.

Injured employee was walking past the Longwall Stageloader toward the face when a piece of coal fell from the roof and hit EE on the back causing EE to fall against the side of the Stageloader near the #1 shield. Received a cut above EE's left eyebrow requiring 5 sutures to close. Returned to work EE's next scheduled shift. No rules or regulations were violated.

Injured employee was operating longwall shields when EE stumbled on a relay bar. EE put out EE's right hand to catch self but EE's hand landed on a bolt plate (Jennmar Mini Monster Mat), causing a laceration. Employee sought medical attention and the laceration was glued closed and bandaged. No rules or regulations were violated and employee has training and experience.

Injured employee was walking through the #5-6 cross-cut at 116 block to get a ventilation tube. Stated EE stepped over a trailing cable and lost EE's balance and fell. Put EE's right hand out to catch EE's fall and hit the coal rib. Has pain and swelling in EE's wrist. Stated EE did not trip or step on cable or obstruction.

Injured employee was dismounting the continuous miner after climbing up onto it to examine the conveyor chain for a possible rock jam. A EE was getting down, EE fell and struck EEs leg against the edge of the machine's cable handler. This caused a deep laceration to EEs left lower leg. Received stitches and given time off work for wound to heal. Has training and 2 years experience.

Employee was carrying a jack down the Slope Belt when EE stepped on loose coal, slipped and fell down. Hit EE's right arm on the floor and the jack EE was carrying struck EE's left side rib area. Employee missed 4 days of work for contusions, pain and soreness. No rules or regulations were violated. Employee has training and experience.

Employee was operating the 6W Longwall Headgate and was pulling excess CO cable when EE tripped on belt structure that had been removed and stacked along the rib. EE fell back and extended left arm out to break the fall. EE landed with arm extended and jammed left shoulder. Was diagnosed with a strain/sprain and possible tear to the rotator cuff.

Injured employee was exiting the vehicle after arriving for work at the Prep. Plant. As EE stepped out of the truck, EE slipped on ice/snow and fell down. Employee suffered a fracture to their upper left arm. Employee had not yet reported for work and was not on the job or performing any work activity. Employee has training and experience.

Injured employee was dragging a ventilation tube and stepped on loose coal/rock causing EE to slip and fall backward, twisting their right knee. Has been initially diagnosed with a knee sprain. No rules or regulations were violated.

Injured employee was using a water hose to wet down the shuttle car haul road on the 6W CM Section. As EE was doing this, EE was walking backward and stepped on a lump of coal and rolled right ankle and fell down. Employee failed to pay attention to where EE was stepping. No rules or regulations were violated and employee has proper training.

Injured employee was attempting to open the 4W Track switch to park the mantrip. The switch lever was stiff to operate so employee was using their right foot to push on the handle to get it to open. When the handle "broke over", employee fell and twisted right knee. No rules or regulations were violated & employee has training/experience.

Upon arriving at work, injured employee was getting out of vehicle and as EE stepped out, EE's foot slipped on the gravel causing left knee to twist. Employee felt pain/soreness in the knee but finished the shift. Employee worked until 6-23-23 when EE began missing work due to this injury.

Employee was using a water hose to clean at the 4 West #1 Belt Drive motor. When EE turned the water on, the pressure caused EE to lose balance and fall down. EE landed on the left knee, hitting it on the concrete pad/metal drive motor base. EE was wearing required PPE, has training and experience in job. No rules or regulations were violated.

Employee was carrying belt rollers from the Longwall belt entry through the L.O.C & placing them in a scoop bucket. While carrying a roller, employee's foot caught a piece of string that was attached to the roller EE was carrying & caused EE to fall. Extended left arm to break fall but injured shoulder when EE landed. Employee has training and experience in task.

Employee was walking down the Slope Belt when EE stepped on some ice and slipped and fell. Ice was present due to a recent waterline break and freezing temperatures. Employee twisted EE's left knee causing a strain/sprain. Missed 2 days of work due to this injury. No rules or regulations were violated and employee has training and experience.

During EE's fall EE grabbed ahold of the rib screen with EE's right hand in an attempt to stop EE's fall. This caused EE to strain EE's right shoulder. Employee stated walkway was clear & EE didn't slip/trip on anything. No rules or regulations were violated & employee has training.

Employee was removing a top roller cradle from the 3 West Belt during belt maintenance. As EE was pulling the cradle out it bumped into something causing it to stop briefly. This caused EE to lose balance & fall while holding the cradle. The cradle landed on EE's left knee causing pain & swelling. No rules/regulations were violated & employee has training/experience in task.

Injured employee was getting out of the operator's compartment of a parked scoop when EE's right foot slipped on the floor causing EE to fall out of the scoop doorway. EE twisted and strained muscles in EE's right hip. Employee began missing work on 6-11-21 due to this injury. No rules or regulations were violated and employee was wearing required PPE.

Employee was walking alongside the left side of the CM when EE tripped on loose coal/rock and fell. EE fell into the side of the CM and hit EE's right wrist against a step on the CM. Had pain and swelling in EE's wrist but finished EE's shift. Sought treatment the following day when EE began missing work. Employee was wearing required PPE and has training and experience in job.

Employee was traveling an intake aircourse with an MSHA Inspector when EE tripped and fell to the mine floor. EE has pain and soreness in right & left shoulder, neck, an abrasion on right elbow, and pain in right knee. The area was examined for tripping hazard/obstructions but none were found. No rules or regulations were violated & EE was wearing required PPE.
