Injured employee was carrying a belt roller with both hands along the 7 North #1 Belt between 24-25 blk. when they tripped on a piece of loose coal and lost balance. They fell against the side of a roller cradle, smashing right hand between the cradle and roller they were carrying. Suffered a contusion & swelling. Was wearing required PPE including metacarpal gloves.

They stuck right forearm out against the rib to catch themselves. Forearm hit a spad that was in the rib causing a small puncture and a cut requiring 3 stitches to close. Employee has training and experience. Returned to work next scheduled shift.

Injured employee was hanging a check curtain on the 7 North Mains CM Section. EE used an 18" high work platform so EE could reach the mine roof. While standing on the platform, EE pulled up on the curtain, lost EE's balance and fell off it, landing on EE's right shoulder. Result was a dislocated shoulder. Employee has training and experience. No rules or regulations were violated.
Injured employee was walking past the Longwall Stageloader toward the face when a piece of coal fell from the roof and hit EE on the back causing EE to fall against the side of the Stageloader near the #1 shield. Received a cut above EE's left eyebrow requiring 5 sutures to close. Returned to work EE's next scheduled shift. No rules or regulations were violated.
Injured employee was operating longwall shields when EE stumbled on a relay bar. EE put out EE's right hand to catch self but EE's hand landed on a bolt plate (Jennmar Mini Monster Mat), causing a laceration. Employee sought medical attention and the laceration was glued closed and bandaged. No rules or regulations were violated and employee has training and experience.
Injured employee was walking through the #5-6 cross-cut at 116 block to get a ventilation tube. Stated EE stepped over a trailing cable and lost EE's balance and fell. Put EE's right hand out to catch EE's fall and hit the coal rib. Has pain and swelling in EE's wrist. Stated EE did not trip or step on cable or obstruction.
Injured employee was dismounting the continuous miner after climbing up onto it to examine the conveyor chain for a possible rock jam. A EE was getting down, EE fell and struck EEs leg against the edge of the machine's cable handler. This caused a deep laceration to EEs left lower leg. Received stitches and given time off work for wound to heal. Has training and 2 years experience.
Employee was carrying a jack down the Slope Belt when EE stepped on loose coal, slipped and fell down. Hit EE's right arm on the floor and the jack EE was carrying struck EE's left side rib area. Employee missed 4 days of work for contusions, pain and soreness. No rules or regulations were violated. Employee has training and experience.
