Injured was attempting to dismount Volvo rock truck with wet muddy boots and EE's foot slipped while climbing down steps causing an unexpected load on EE's left shoulder resulting in a sprain/strain type injury. Injured started missing work on 3/13/26 and was put off work pending an MRI. No problems with equipment or compliance issues, training or PPE.

Injured was hanging a run through curtain up in #2 entry of St Leo Mains 131-132 block, standing on the third rung of a 4' ladder leaning off the ladder to install a nail through the curtain when EE fell off the ladder and bruised EE's back when landing on the mine floor. No problems with equipment or compliance issues, training or PPE.

Injured standing in first crosscut outby 8 west equipment doors near the end of loose take up rails while a scoop was unloading a double stack of take up sections on top of the loose rails. Duckbill drug across a loose rail, kicking it up when it got to the end of the rail striking injured, knocking EE down and landing on EE resulting in a fractured vertebrate.

Having trouble fixing an air tugger, EE was on the platform and coworker just looked at them and had eye contact. Coworker turned to keep working with third person and they heard a noise and EE was on the ground. No one knows exactly what happened. The assume that the safety catch for the drill steel came out.

While making preshift examination on 6 West belt, just outby the middrive, the examiner miss stepped on left foot, stepping on its side and rolling the ankle, resulting in a fracture to the left fibula. While falling they struck left elbow on belt structure resulting in a chipped bone. No problems with equipment or compliance issues, training, or PPE.

While walking in the Miracle Run Loaded Track injured tripped and fell, striking left side of ribs against mine rib, causing a contusion. Examination of area revealed no tripping hazard, and EE does not know what EE tripped over (maybe track rail). No problems with equipment or compliance issues, training or PPE.

The injured was hanging belt structure using a step ladder in 7W Belt Trench. A scoop in an adjacent entry caught a cable running through the trench. The cable pulled contacting the ladder knocking the EE to the ground. EE began missing 5/1/24.

The EE was climbing down off of D-7 dozer. The injured slipped on the step at the blade arm and fell. EE's left ear struck the dozer cat, causing a laceration. EE received 4 stitches, and did not miss any scheduled shifts.

The EE was carrying Cement blocks, when EE tripped over the plastic banding that secures the bundle of blocks. The EE caught self before falling to the ground and felt a pain in lower back. The EE was wearing PPE. Started missing work 5-18-23, Returned 5-23-23.

The injured employee tripped over the cable handler of a loading machine, twisting a knee and striking lower leg against a shuttle car waiting to be loaded. This resulted in a torn ligament and a fracture to the lower leg. It did not involve any compliance issues, job skills, proficiency, training, PPE or protective devices.

The EE had just climbed over the GOB plate on the LW face. As EE was stepping out into the TG entry, EE's left foot went in a hole in the pavement. This caused EE's ankle to roll. EE suffered a fracture above the left ankle.

While employee was working in a belly pan replacing discharge rubber someone started a water pump which started filling belly pan with water. As EE was evacuating the belly pan EE slipped and aggravated right wrist that had underwent carpal tunnel surgery on 7/11/22. No problems with equipment or compliance issues, training or PPE.

EE was walking past #4 shield on the LW face. EE's toe caught a D-ring on the relay bar, and EE tripped. As EE was falling, EE reached with right hand and was able to grab a chain. This caused the EE to strain shoulder as EE felt a pop.

The EE was in the process of changing a hose on the head of the miner in the 9 North Mains Section. The cob was already removed, and as the EE began to get up, EE slipped and fell against the head. This caused a laceration to EE's left wrist. EE received 6 stitches, and began missing work on 7/28/22.

When EE stepped out of a scoop EE felt EE's left knee give out, and this occurred several more times during the shift, resulting in a strain/sprain type injury to the left knee. The injured EE was dragging a rock dust hose. While dragging the hose, the EE tripped over the hose. When EE fell to the ground EE struck EE's back on the track rail. Incident occurred in the chutes area of Sugar Run. EE began missing work 3-21-22.

The EE was pulling a whip line attached to a breast drill. The injured stepped down on a piece of coal and fell, injuring right knee. The EE worked until was scheduled for surgery on 5/18/22. EE began missing on that day.

EE was the utility man on the 4W mining section. EE had climbed on the miner to hang up a dog chain for a ventilation tube. The EE slipped off the miner, catching left arm on the rib drill, injuring the left shoulder. EE began missing work 2/16/22.

The injured EE fell in the shower after tripping on their sandal. The EE lost balance, fell forward, and struck their face on the shower nozzle. The EE received stitches above eye and a fracture to their face. EE began missing work 11/1/21.

EE was knelt down putting together a dust hose, when EE lost balance. As the injured fell, EE put hand up to catch self. The EE's right ring finger came into contact with a bottom roller. EE's finger was pulled and caught between the belt and bottom roller.

The employee was examining 6 ML Belt. EE ducked under a waterline, as EE did, EE stepped in a ditch twisting right knee. The employee was scheduled off and then saw a specialist on 7/23/21. At this time EE began missing work.

Employee slipped on stairs in the prep plant going down from the 10th floor to the 9th floor. When the employee slipped they grabbed the handrail and strained left shoulder and also over extended and strained right hip. Employee was wearing the proper PPE. Employee did not start missing work until 8/2/2021.
