Showing all 5Operator fell due to loosing grip and footing while climbing down the stairs of Front Loader. Recent rainfall caused wet/muddy conditions in the job site area which contributed to the incident. Operator sustained minor injuries to leg, hip, and elbow due to the incident.
EE stated EE had stepped up on top of 3 pallets to hand a bulk bag on the packer to fill. EE's knee just gave out. EE was unable to walk and the ambulance was called.
Employee was walking towards front office door, after exiting front office tripped over a shipping box employee landed on left knee and twisted leg. Also bruised right thumb when hand hit the floor.
EE was teaching a temp to run the robot, as EE began down the stairs EE started coughing and missed the last step which led to a fall on EE's right side, in which EE rolled/twisted EE's left ankle. EE refused medical treatment initially. One Dec 28th 2023, EE called and stated EE's ankle was hurting and requested medical treatment.
