The employee was descending the ladder from the top deck of the cone when a hand slipped approximately midway down, causing them to fall to the deck below. Reported to management 10/30 when diagnosed with a hernia.

On 2-7-24 between 5 and 6 am. employee was checking the 57-rock stacker during routine monitoring of plant. EE noticed that belt #1 feeding the 57-rock stacker had stopped. EE started making way to the control panel to shut off belt to prevent overflow. On EE's way to the control panel to shut it off, EE stepped wrong hurting their left foot around the achilleas area.

On 11/22/23 at 11:40 am EE was climbing down the screen deck ladder. As EE was climbing down the ladder EE's foot slipped due to mud on EE's boot causing EE to fall off the ladder 5-6 feet to the ground injuring EE's back. EE was unable to prevent the fall because EE was not utilizing 3 points of contact due to having a wrench in one of EE's hands while climbing down.

EE was on top of a conveyor hooking up chains to the lifting eyes to lift the conveyor with the 988k Loader. EE signaled to the loader to put pressure on the chains and then unhooked the fall protection to be able to climb down. The chains that were attached then slipped causing the employee to fall off the side of the conveyor 10 feet to the ground.
