IN AN EFFORT TO REACH PIPE FLANGE INJURED PLACE MAJORITY OF HIS WEIGHT ON PORTION OF LADDER EXTENDING ABOVE PIPE CAUSING FOOT OF LADDER TO LIFT ALLOWING LADDER TO SLIDE OFF PIPE.
