EE and another employee were replacing the protector flap on the 1955 Kohlman Conveyor. It had rained the night before and the conveyor was wet. EE's foot slipped off the frame of the conveyor EE was standing on causing EE to fall and break EE's wrist. EE was wearing fall protection.

Employee was climbing out of a front end loader when the last step broke causing the employee to stumble and fall on the ground. *Employee notified office on 7/12/2021.
Heavy overnight rains caused excess mud in the equipment parking area. As employee walked to assigned front end loader, left leg slid out in front of them. EE regained control without falling but hurt a knee. Restrictions include partial weight bearing through left lower extremity avoid flexing >90 degrees.
Contractor was unloading a crate of crusher parts from the pick up bed when a board on the crate broke. EE slipped on the wet, plastic bed liner, fell on a skid steer bucket. EE drove self to the nearest hospital where EE was treated for a bruise and released.
