While stepping onto a 2' stepladder, employee fell backwards onto concrete floor, injuring their left hand.
The miner was climbing down from a scaler and missed the last step falling backward onto a nearby pickup truck hitting EE's back.
EE slipped while exiting a CAT 980 end loader and caught self with the right hand causing a tear of the left bicep.
While removing motor and motor mounts, the motor mounts attach to the catwalk, leaving a space approx 1.5'x1.5'. Employee was carrying 2'x2' piece of wood to place over space. Employee inadvertently stepped into the space scraping shin and thigh. After EE reported to supervisor EE refused medical treatment the date of this accident.
Employee was walking to the truck line from the shop building, slipped on the snow covered ground EE was walking on and fell. EE injured their lower back and has pain in the right shoulder.
The employee was climbing up ladder in front of haul truck cab, hand slipped causing ee to lose 3 points of contact and fell to the ground landing on feet.
Employee reported to Safety Department his knee was bothering him with considerable pain.
Repair work was being done on a tail-pulley at conveyor #C-390; while handling the tail-pulley the employee slip in some mud and the tail-pulley struck his right ring finger causing a crushing and lacerations injuries with tendon damage.
While doing maintenance on a Euclid R-50 Hual Truck and using a 1'x1'x1' (+/-) block of wood as a step turned to step off/down from the block and rolled right ankle resulting in a sprain.
Employee was removing a bushing from a phasing arm of a horizontal screen (P-009). Employee apparently slipped and fell 20 feet to the ground. Employee was not wearing fall protection provided by the Employer. Injuries are unknown to Employer at this time, but appear to be not life threatening, nor permanently disabling.
EE was assisting with repairs. Using a wrench, the wrench slipped, ee slipped and fell backwards from a sitting position onto a rock on concrete. EE incurred bruised muscled in back. EE had a preexisting herniated disk in back, He was currently completing physical therapy so ee was taken to hsp for observation. EE was instructed to rest and stretch the muscles.
Weather was clear and mild with no wind no contributing factor obvious. Worker slipped, tripped or fell from Primary Crusher. Worker suffered trauma to head, neck and fractured his right wrist.

WHILE ACCOMPANYING AN MSHA INSPECTOR ON A ROUTE INSPECTION OF AREA, I SLIPPED ON GROUND COVER AND FELL, STRIKING MY LEFT SHOULDER/ARM ON THE GROUND. NO INJURY NOTED AT THAT TIME. IN JULY I HAD AN M.R.I. AND IT WAS DISCOVERED THAT I HAD A TORN ROTATOR CUFF. THIS AFTER 3 MONTHS HAD PASSED.
WHILE USING BOOM TRUCK S-68 TO MOVE STEPS AND AFTER WHILE THE BOOM WAS UNHOOKED NOT UN LOAD. THE BOOM FELL FROMT HE TRUCK BOLT METAL FATIGUE EE JUMPED FROM APPROX. THE AREA OF THE TRUCK RUNNIN G BAORD TO AVOID BEING STRUCK BY THE BOOM. IN DOING SO HE STRUCK THE GROUND WITH HIS SHOULDER. FACE AND HAND.
EMPLOYEE FAILED TO USE PROPER FALL PROTECTION. WHILE HELPING REPLACE A BURNT OUT ELECTRIC MOTOR, HE FELL STRIKING THE BED OF MACK DUMP TRUCK, INFLICTING MULTIPLE FACIAL LACERATIONS THAT REQUIRED STITCHES.
