On 9/28/25, EE slipped in a small hole and injured EE's knee while getting out of EE's vehicle for work. EE reported it to team lead at 12:00 PM; TL informed supervisor at 6:24 PM. EE requested medical attention the next day, was evaluated, given medication and work restrictions that didn't affect equipment duties. Released on 10/2/25.

Upon shift start, EE walked into the GN Mill Bld MCC room, and seemingly got into a physical altercation with an EE. Assumption is that IE was pushed or struck and fell against a main breaker bucket damaging some of the plate metal. IE diagnosed with ac joint separation to (L) shoulder, minor cut on mouth, ear & back. Also syncope at some point. Other EE no injuries.

EE was spotting for railcar mover and slipped and hit head. EE left spotting activity to talk with RR guy in truck parked nearby. They decided to go over and talk with guy about railcars. The stepped in a water puddle and slipped and fell, bumping head on ground. EE received medical attention(6 staples to close cut)all scans/tests negatives at hospital and released same night.

EE has ground level fall while exiting from Bdg#2. EE stepped off traction grating and onto slick pavement from rain event prior, causing EE to fall forward and landing on right hand and knee. EE diagnosis: acute right knee sprain

The operator stated: climbed on the side of the forklift & was standing on the tire of forklift releasing the latch by hand on the self-dumping trash can when teammate slipped & hurt foot. Teammate stated did not fall to the ground. Teammate has a dislocated big toe on left foot. Surveillance video shows no such event. Awaiting teammate to work for further investigation.

At approx. 10:41pm prior to clocking in, employee stated to have said to the teammates in the Control Room, that they had fallen but told everyone they were fine. EE then went on to work the entire shift without issue. EE called Supervisor at about 1215 pm on July 20, 2021 and during conversation the teammate revealed to that they slipped and fell.

Two (2) workers were lifting a core drill onto the back of a pickup truck, when one slipped and fell with the core drill landing on EE's leg. The ground was slippery from the wet guragum on the ground. The worker's knee had a small laceration and was bruised.

Teammate decided to cross 45 degree slope at truck scale exit. Teammate slipped and fell while descending slope, EE got up and came inside and reported incident to Traffic Manager

Clay was wet from rain. Walking. Pain to right knee and left wrist from fall.

Dismounted loader to cut off conveyor at stop button panel & slipped in mud, having same level fall. Left knee hurting, so taken to ER. Diagnosis left knee pain - given ace bandage, medication & told to elevate & rest. Released for 11-7-18.

Employee was stepping down off of fuel trailer using left foot & left foot rolled. Diagnosis: left ankle sprain.

EE lifting a roll of film, heard a pop & fell to floor. ER physician found no injuries & referred EE to family physician. WCI physician expressed concern about headache and imposed work restrictions pending follow up.

Standing on lift table pulling stack of jugs (empty) up roller bed, when right foot slipped on roller. He fell backwards landed on right lower back & forearm. Strain & bruised.

Walking around elevator & tripped on clay chunks causing a same level fall. Landed on right side arm and hip(radio on hip). On 5-8-15 doctor diagnosed as contusion sent back with no restrictions. On 5-12-15 X-ray report showed wrist having a minimally displaced fracture through the triquetrum with soft tissue swelling - referred to Orthopedic doctor.

EE retrieving parts & stepped on bin divider on floor w/ right foot. Divider slid forward & he fell on left knee(same level). Had pain & swelling x-ray shows no fracture. EE on restricted duty.

EE had just applied LVM on slippery area by OMW & was leaving area when he slipped and fell.He tried to catch himself with rt arm injuring his rt shoulder & arm. Diagnosis: rt shoulder and arm contusion. Light duty until next week.

Stepping down on step ladder, lost her balance & twisted left knee. Inflammation of left knee. Restricted, wear immobilizer, no work for 4 days.

Washing down a muddy area to get a lab sample when he slipped and had a same level fall on left side no injury. Later, on 12-22-11 sent to doctor with backache no restrictions. 1-4-12 went back to doctor & was told no work until consult with Orthopedic doctor. Became a reportable on 1-4-12.

Stood up from opening hatch on railcar and started walking and rolled his left ankle causing him to fall (same level fall). Left ankle sprain. Restricted can operate forklift, but no climbing stairs.

EE reported he fell walking down stairs & landed on left knee. Diagnosis left knee contusion. Since date of accident the EE has performed his regular job duties. The treating physician has now prescribed physical therapy based on a recent MRI. 1st appt is 8-9-11.

Walking and tripped on strap & debris fell on left knee bumping it & head(hard hat on) on pallet of empty bulk bags. Inflammation in neck & soreness in left shoulder and knee. Able to work with restrictions.

Thawing frozen lines & changing motor. While changing motor, left foot slipped. Okay that day, but next day foot swollen & hurting. Left ankle sprain, grade 2. Restricted with air brace & crutches.

At truck scales and opened load cell for cleaning. Pulling fire hose and stepped into the hole he just opened. Stated he was fine. On 12-23-09 came to us and stated he is experiencing pain so he was sent to doctor. Diagnosis ankle sprain. Gave him ankle brace and restrictions.

While checking his area fell and hurt back. Lumbosacral right contusion, pain. Restricted.

Employee was walking in mud with rubber boots when boot got stuck causing him to fall. Tried to break fall with hands. Dislocated 5th digit left hand & contusion to right hand.

EE was ascending ladder and struck left knee on safety cage brace. Contusion of left knee, knee immobilizer, and on light duty.

Injury was checking clay falling on CV25 belt. He started towards spray system to check tips and slipped on wet surface. He fell injuring his left knee. Diagnosis - strain/sprain & joint mouse of left knee. He is scheduled for surgery.

Employee suffered a same level fall, he tried to extend arms to brace against fall. His injury was bilateral elbow fractures. Employee expected to have surgery Monday Nov 17, 2008.

He was climbing ladder to disintegrator to clean out chute. Stepped on platform, feet shot out from under him causing him to fall 5 to 6 feet. He landed on his back, on concrete. Diagnosis: left rotator cuff tendinitis. No work for 1 week.

Employee was walking from breakroom going to catch a product sample and slipped and fell in the travelway (rained during the night). He sustained an avulsion fracture to his left wrist. He is working with restrictions and wearing a splint.

Employee walked down a flight of stairs. It was wet and muddy at the bottom of the stairs due to hard blowing rain storm. Employee slipped as he stepped off of last step and fell at floor level landing on his right shoulder. Employee complained of pain in the right shoulder.

EMPLOYEE WAS WALKING AND STEPPED ON A WET MAT. HE SLIPPED AND FELL ON A METAL GRATING.

EE slipped on wet surface while walking on a cat walk. struck left knee on metal cat walk, causing bruise. ee was reassisgned to rotary kiln control room until condition improves.

CLIMBED SAFETY LADDER TO CLEAN DRYER, STEPPED DOWN LADDER AND HAD MUD ON SHOES AND LADDER WAS SLICK, HIT LEFT KNEE ON IRON LADDER.

EMPLOYEE WAS MOVING COVER ON PUMP TRUCK IN PREPARATION FOR LOADING CLAY AND FELL.

EE STACKING BAGS AND STEPPED ON A BAD PALLET AND FELL, CATCHING HIMSELF WITH HIS HANDS.

THE EE WAS EXITING THE FRONT END LOADER WITH GREASE ON HIS HANDS. HIS FOOT SLIPPED AND HIS HAND SLIPPED FROM THE RAIL, CAUSING HIM TO FALL TO THE GROUND.

EE WAS DESCENDING A FLIGHT OF STAIRS AFTER A RECENT RAIN AND HIS FOOT SLIPPED ON A WET SPOT. HE REACHED OUT TO BREAK HIS FALL AND SPRAINED HIS RIGHT WRIST (PER MEDICAL EXAM ON 2/19). ON 3/5/02 HE WAS RE-EXAMINED AFTER COMPLAINING OF FURTHER DISCOMFORT. ON 3/21 HE WAS EXAMINED AS A ROUTINE FOLLOW-UP AND ON 3/25 WAS REFERRED TO PHYSICAL THERAPY.

EE MOVING TOW BEHIND WELDER INTO POSITION WITH TRUCK HITCH. HIS HAND WAS PLACED ORDER WELDER HITCH, HE SLIPPED AND CAUGHT FINGER UNDER WELDER OR TRUCK HITCH CAUSING AMPUTATION OF RIGHT RING FI NGER AT TIP.

WALKING DOWN LOADING RAMP SLIPPED, BECAUSE OF MUD ON SHOE AND FELL DOWN. LEFT KNEE SWOLLEN.

EE WAS TAKING DOOR OFF OF DUST BIN, DUST HAD BUILT UP BEHIND DOOR UNKNOWN TO EE. WHEN DOOR WAS LOOSENED DUST ESCAPED AND BROUGHT VISIBILITY TO ZERO. AS EE CLIMBED DOWN LADDER TO ESCAPER DUST, HE SLIPPED OFF A LADDER RUNG AND WHEN HE LANDED ON THE CONCRETE HE HIT WITH HIS LEFT SHOULDER FIRST, CAUSING INJURY.

OPERATOR WAS WALKING FROM EQUIPMENT. SOFT GROUND GAVE WAS CAUSING OPERATOR TO LOSE BALANCE. OPERATOR FELT SOMETHING SHIFT IN HIS NECK AND SHOULDERS. PAIN WORSENED AS TIME PROGRESSED. REPORTED AND SOUGHT MEDICAL ATTENTION THE NEXT DAY. MEDICATION PRESCRIBED.

EE WAS PULLING GREASE HOLE ACROSS THE GRD THAT HAD BECOME SLIPPERY DUE TO RECENT RAIN SHOWER. HELOST HIS FOOTING & FELL INJURING HIS BACK BUT CONTD TO WORK AFER HIS FALL. HE REPORTED INJURY SE VERAL WKS LATER (12/01/00) AND REQUESTED MEDICALATTENTION (PLEASE SEE ATTACHED DOCUMENT)

EE WAS ATTEMPTING TO RESTACK A PALLET OF CAT LITTER THAT WAS IN THE PROCESS OF BEING STRETCH WRAPPED FOR SHIPPING. HE HAD PLACED ONE FOOT ON THE TURNTABLE AND THE OTHER ON THE CONVEYOR ROLLER WHEN HE TEMPORILITY LOST HIS FOOTING BUT CAUGHT HIMSELF BEFORE FALLING. HE BEGAN TO HAVE BACK PAIN AND WAS TAKEN TO THE COMPNAY DR AND EXAMINED.

EMPLOYEE WAS WALKING THROUGH HIS DEPARTMENT WHEN HIS FEET BECAME INTANGLED IN GREEN PALLET STRAPPING CAUSING HIM TO TRIP AND FALL. HE EXTENDED HIS LEFT ARM TO BREAK HIS FALL AND SPRAINED HIS LOWER WRIST AND HURT HIS THUMB. HIS WRIST WAS PLACED IN A SPLINT.
