On 9/28/25, EE slipped in a small hole and injured EE's knee while getting out of EE's vehicle for work. EE reported it to team lead at 12:00 PM; TL informed supervisor at 6:24 PM. EE requested medical attention the next day, was evaluated, given medication and work restrictions that didn't affect equipment duties. Released on 10/2/25.

Showing all 45Upon shift start, EE walked into the GN Mill Bld MCC room, and seemingly got into a physical altercation with an EE. Assumption is that IE was pushed or struck and fell against a main breaker bucket damaging some of the plate metal. IE diagnosed with ac joint separation to (L) shoulder, minor cut on mouth, ear & back. Also syncope at some point. Other EE no injuries.
EE was spotting for railcar mover and slipped and hit head. EE left spotting activity to talk with RR guy in truck parked nearby. They decided to go over and talk with guy about railcars. The stepped in a water puddle and slipped and fell, bumping head on ground. EE received medical attention(6 staples to close cut)all scans/tests negatives at hospital and released same night.
EE has ground level fall while exiting from Bdg#2. EE stepped off traction grating and onto slick pavement from rain event prior, causing EE to fall forward and landing on right hand and knee. EE diagnosis: acute right knee sprain
The operator stated: climbed on the side of the forklift & was standing on the tire of forklift releasing the latch by hand on the self-dumping trash can when teammate slipped & hurt foot. Teammate stated did not fall to the ground. Teammate has a dislocated big toe on left foot. Surveillance video shows no such event. Awaiting teammate to work for further investigation.
At approx. 10:41pm prior to clocking in, employee stated to have said to the teammates in the Control Room, that they had fallen but told everyone they were fine. EE then went on to work the entire shift without issue. EE called Supervisor at about 1215 pm on July 20, 2021 and during conversation the teammate revealed to that they slipped and fell.
Two (2) workers were lifting a core drill onto the back of a pickup truck, when one slipped and fell with the core drill landing on EE's leg. The ground was slippery from the wet guragum on the ground. The worker's knee had a small laceration and was bruised.
Teammate decided to cross 45 degree slope at truck scale exit. Teammate slipped and fell while descending slope, EE got up and came inside and reported incident to Traffic Manager

EE retrieving parts & stepped on bin divider on floor w/ right foot. Divider slid forward & he fell on left knee(same level). Had pain & swelling x-ray shows no fracture. EE on restricted duty.
EE had just applied LVM on slippery area by OMW & was leaving area when he slipped and fell.He tried to catch himself with rt arm injuring his rt shoulder & arm. Diagnosis: rt shoulder and arm contusion. Light duty until next week.

EE WAS DESCENDING A FLIGHT OF STAIRS AFTER A RECENT RAIN AND HIS FOOT SLIPPED ON A WET SPOT. HE REACHED OUT TO BREAK HIS FALL AND SPRAINED HIS RIGHT WRIST (PER MEDICAL EXAM ON 2/19). ON 3/5/02 HE WAS RE-EXAMINED AFTER COMPLAINING OF FURTHER DISCOMFORT. ON 3/21 HE WAS EXAMINED AS A ROUTINE FOLLOW-UP AND ON 3/25 WAS REFERRED TO PHYSICAL THERAPY.
EE MOVING TOW BEHIND WELDER INTO POSITION WITH TRUCK HITCH. HIS HAND WAS PLACED ORDER WELDER HITCH, HE SLIPPED AND CAUGHT FINGER UNDER WELDER OR TRUCK HITCH CAUSING AMPUTATION OF RIGHT RING FI NGER AT TIP.

OPERATOR WAS WALKING FROM EQUIPMENT. SOFT GROUND GAVE WAS CAUSING OPERATOR TO LOSE BALANCE. OPERATOR FELT SOMETHING SHIFT IN HIS NECK AND SHOULDERS. PAIN WORSENED AS TIME PROGRESSED. REPORTED AND SOUGHT MEDICAL ATTENTION THE NEXT DAY. MEDICATION PRESCRIBED.
EE WAS PULLING GREASE HOLE ACROSS THE GRD THAT HAD BECOME SLIPPERY DUE TO RECENT RAIN SHOWER. HELOST HIS FOOTING & FELL INJURING HIS BACK BUT CONTD TO WORK AFER HIS FALL. HE REPORTED INJURY SE VERAL WKS LATER (12/01/00) AND REQUESTED MEDICALATTENTION (PLEASE SEE ATTACHED DOCUMENT)
EE WAS ATTEMPTING TO RESTACK A PALLET OF CAT LITTER THAT WAS IN THE PROCESS OF BEING STRETCH WRAPPED FOR SHIPPING. HE HAD PLACED ONE FOOT ON THE TURNTABLE AND THE OTHER ON THE CONVEYOR ROLLER WHEN HE TEMPORILITY LOST HIS FOOTING BUT CAUGHT HIMSELF BEFORE FALLING. HE BEGAN TO HAVE BACK PAIN AND WAS TAKEN TO THE COMPNAY DR AND EXAMINED.
EMPLOYEE WAS WALKING THROUGH HIS DEPARTMENT WHEN HIS FEET BECAME INTANGLED IN GREEN PALLET STRAPPING CAUSING HIM TO TRIP AND FALL. HE EXTENDED HIS LEFT ARM TO BREAK HIS FALL AND SPRAINED HIS LOWER WRIST AND HURT HIS THUMB. HIS WRIST WAS PLACED IN A SPLINT.
