Employee was descending dozer. EE stepped from dozer to stepladder. The ladder went sideways. Employee tried to catch self by grabbing hose on the dozer's lift cylinder, but EE's hand slipped, causing EE to hit the left side of head against the dozer filter and fell to the ground.

Showing all 76'1", 430lb employee was performing morning mobile equipment servicing duties. He was dismounting passenger-side ladder of 988 endloader. As he applied his weight to the last rung, the ladder gave way. He did not fall. He was able to maintain 3 pt contact w/left leg extended. Emp. claimed he heard a "pop" in his left knee. Medical exam determined sprain/tissue injury to left knee.
Employee slipped on a piece of snow covered plexi-glass. He fell on his right hip.

The injury was the result of the employee not being adequately aware of his environment and incorrect foot placement. No equipment was involved, employee was wearing required PPE. Employee was familiar with job being performed.
