Employee was working on a haul truck, once finished employee was carrying tools in one hand & pushing the oil pan with the other. The employee lost balance & tripped landing on right elbow. The employee was taken to the hospital where employee was examined. The employee was advised to rest, ice, & elevate. Employee was placed on light duty until 6/18/23. No broken bones from xray.

Showing all 12Contract, over-the-road driver fell from their trailer while tarping the load. Driver was tied off, but as driver fell driver scraped right lower leg on truck or catwalk, had a puncture wound on the left lower leg possibly by truck, catwalk, or a bungee cord being used to secure tarp, and scratched left elbow on the truck or catwalk.
Employee was making sure that a tie-down strap was secure to truck. When ee pulled on the strap, it was loose, ee lost balance and fell. As ee fell the strap caught and pulled arm. This caused rotator cuff on right shoulder to tear.
Employee was painting outside of hopper when they lost balance and fell. The fall restraint lanyard expanded to slow the fall. Then the tie off point gave way allowing them to impact the ground causing contusion to right elbow, and sprain the right knee.
While going down stairs, employee tripped, tried to catch himself on handrail and injured right shoulder.
Getting out of dump truck, driver slipped on steps and fell to ground hitting his left hip. Steps were wet due to rain.
Tripped over hose and broom while washing truck #130, fell between truck and cross member.
Was getting on 345 excavator stepped on first step with hand on hand rail felt pain in back.
Employee was stepping out of Komatsu skidstear loader twisted let knee.
Changing belt line, slipped on piece of belt being removed.
WAS WALKING FROM TRUCK CAB BACK TO LOAD TRAILER. SLIPPED AND FELL BETWEEN DOCK AND TRAILER BED, TWISTING RIGHT KNEE.
EMPLOYEE WAS CLIMBING OUT OF LOADER STEP ON GROUND AND CREELED LEFT ANKLE.
