HE WAS GETTING OUT OF THE BUCKET TRUCK, SLIPPED ON SOME SAWDUST AND OIL AND FELL. HIS HEAD HIT A HOOK CAUSING A LACERATION. HIS BACK HIT A SHARPCORNER OF AN OIL TANK, IT SKINNED HIS RIGHT ARM BELOW THE ELBOW, RIGHT HIP, RIBS AND LOWER BACK WERE ALSO INJURED.

THE STEPS OF THE SHOP WERE COVERED WITH ICE AND SNOW. AS THE EMPLOYEE STEPPED OUT OF THE SHOP HE SLIPPED AND FELL TO THE GROUND. IT TWISTED HIS RIGHT KNEE.
